血清胰岛素样生长因子及其结合蛋白诊断儿童生长激素缺乏症的价值

摘要 为了解血清胰岛素样生长因子 1(IGF- 1)、胰岛素样生长因子结合蛋白 - 3(IGFBP- 3)浓度与生长激素缺乏症 (GHD)患儿生长激素 (GH)激发试验中血清生长激素峰值的关系 ,以确定血清 IGF- 1、IGFBP- 3浓度诊断GHD的价值 ,为其代替 GH激发试验提供依据 ,选择 GHD患儿 6 2例 (男 39例 ,女 2 3例 )为 GHD组 ,6 0例健康儿童 (男 38例 ,女 2 2例 )为对照组。分别用放射免疫分析 (RIA)法、免疫放射分析 (IRMA)法检测 GHD组血清IGF- 1、IGFBP- 3浓度 ,同时做 GH激发试验 ,测定血清 GH峰值 ,并比较其与 IGF- 1、IGFBP- 3的关系 ;测定对照组血清 IGF- 1、IGFBP- 3值。结果显示 ,GHD组血清 IGF- 1、IGFBP- 3均显著低于对照组 (t分别为 3.116、11.5 79,P均 <0 .0 1) ;GHD组血清 IGF- 1、IGFBP- 3浓度与 GH激发试验中的 GH峰值呈显著正相关 (r分别为 0 .331、0 .347,P均 <0 .0 1) ;GHD组血清 IGF- 1、IGFBP- 3降低的阳性率分别为 97.5 8%、98.38% ,与激发试验的阳性率(10 0 % )比较无统计学意义 (χ2 分别为 3.0 74、2 .0 33,P均 >0 .0 5 )。表明血清中 IGF- 1、IGFBP- 3浓度检测对诊断GHD有重要价值 ,认为检测血清中 IGF- 1、IGFBP- 3浓度可以替代 To assess the relationship between the serum insulin like growth factor 1(IGF 1),insulin like growth factor binding protein 3(IGFBP 3) levels and the peak growth hormone(GH)concentrations by GH provoke testing of the children with growth hormone dificience(GHD).All the children were divided into two groups:GHD group 62 (male 39,female 23);Control group 60(male 38,female 22).The serum IGF 1,IGFBP 3 levels of all children were measured by RIA,IMRA respectively,and determining the peak GH levels by RIA.The differention between two groups were comparedu sing Student's test,the relationship of the serum IGF 1,IGFBP 3 and the peak GH levels using relative regress.Results showed that the serum IGF 1,IGFBP 3 levels of the children with GHD were lower significantly than control group(t=3.116,11.579,P<0.01,respectively).The sreum IGF 1,IGFBP 3 levels with the peak GH levels were positive relation in GHD group.This suggests that the GH provoke testing can be insteaded by the serum IGF 1,IGFBP 3 levels when the child is suspected GHD.
