摘要
目的:探讨急性出血坏死性胰腺炎腹水与血清IL-6的关系及肝脏对其所起的作用。方法:40只大鼠随机分为正常对照组、无门腔静脉分流腹水注射实验组、急性出血坏死性胰腺炎组、分流对照组及分流后腹水注射实验组。每组动物均为8只,手术后4 h从心脏采血,测定血清中IL-6含量。另制作急性出血坏死性胰腺炎大鼠6只,4 h后抽取腹水、门静脉及腔静脉血,测定内毒素含量。结果:急性出血坏死性胰腺炎腹水中内毒素含量明显高于门静脉及腔静脉。无门腔静脉分流腹水注射实验组、门腔静脉分流腹水注射实验组血清IL-6水平明显高于正常对照组、分流对照组和急性出血坏死性胰腺炎组,差异有显著意义(P<0.01)。而前两组之间无明显差异。结论:急性出血坏死性胰腺炎腹水中有高浓度的内毒素,可引起促炎递质IL-6升高。且不受肝脏作用的影响,早期防止或减少胰性腹水的吸收对减轻急性出血坏死性胰腺炎造成的全身损害可能有益。
Objective: To study the relationship between the ascites and serum IL-6 level of the acute necroting pancreatitis [ANP] rats. Methods: Forty Wistar male rats were divided into 5 groups: control group, ascites injecting group, ANP group, portacaval shunt group and post-portacaval shunt ascites injecting group. Blood was collected from the heart in each group 4 hours after operation and the serum IL-6 levels were measured. In addition, in ANP group ( n - 6), the endotoxin in ascites, blood of portal vein and caval vein were also measured. Results: The IL-6 levels of ascites injecting group and post-portacaval shunt ascites injecting group were higher than those of the other groups ( P < 0. 01). The difference between the former two groups was not significant. The levels of endotoxin in ascites of ANP group were much higher than the levels in portal vein and caval vein. Conclusion: The high levels of endotoxin in ascites of ANP group can increase serum level of IL-6. It is beneficial to early prevent and reduce the endotoxin absorption in ascites, which may alleviate systemic impairment as a result of ANP.
