摘要
目的:探讨高血压左心室肥厚(LVH)与血清瘦素(Leptin)、一氧化氮(NO)水平的关系及缬沙坦(val-sartan)对其的影响。方法:选取60例原发性高血压(EH)并LVH患者(Ⅰ组),60例EH无LVH患者(Ⅱ组);分别进行血清Leptin、NO水平测定,并将Ⅰ组的左室重量指数(LVMI)与Leptin、NO进行相关分析。Ⅰ组给予24周valsartan干预后,复测血清Leptin、NO水平。血清Leptin测定采用放免法,血清NO水平测定采用硝酸还原酶法。结果:Ⅰ、Ⅱ组血清Leptin、NO水平比较差异均有统计学意义(P<0.01);LVMI与Leptin呈正相关、与NO呈负相关;Ⅰ组经valsartan24周干预前后比较各指标水平差异均有统计学意义(P<0.01)。结论:Leptin、NO分别是促进和抑制LVH的重要物质;缬沙坦降低血清Leptin水平、升高血清NO水平可能是其抗LVH的机制之一。
Aim: To investigate the relationship between hypertension with left ventricular hypertrophy ( LVH) and the serum levels of Leptin and nitric oxide ( NO) as well as the influence of valsartan on them. Methods: The serum levels of Leptin and NO were simultaneously measured in 60 cases of essential hypertension ( EH) with LVH ( group I ) and 60 ca ses of EH without LVH ( group Ⅱ ) respectively. Correlative analysis of LVMI and serum Leptin and serum NO in group Ⅰwas carried out. The patients of group Ⅰ took valsartan over 24 weeks and the levels of Leptin and NO were measured a-gain. Serum Leptin levels was measured by radioimmunoassy and serum NO level was assayed with nitric-acid-reductase. ReSUltS: Serum Leptin level was higher in group Ⅰ than that of group Ⅱ (P < 0. 01) , but serum NO level was lower in group Ⅰ than that of group Ⅱ (P <0. 01 ) . LVMI was positively correlated with Leptin, but negatively correlated with NO; there was significant difference in the serum levels of NO,Leptin and the level of LVMI before and after treatment in group Ⅰ (P <0.01) . Conclusion :Serum Leptin and serum NO are the important substances which are promoting and inhibiting factors on LVH respectively ; one of the mechanisms for preventing LVH of valsartan is to reduce the serum level of Leptin and ascend the level of NO.
