摘要
目的探讨超声检查联合X线摄片乳腺导管造影对乳管内占位性病变的诊断价值。方法回顾性分析2010年6月至2015年6月我院收治的93例乳管内占位性病变患者的超声和X线摄片乳腺导管造影的影像学资料,对单项检查和联合检查的结果进行比较。结果超声检查发现乳管内占位性病变的检出率为82.80%(77/93),漏诊率为17.20%(16/93),发现乳管内恶性占位性病变的检出率为40.00%(4/10),漏诊率为60.00%(6/10);X线摄片乳腺导管造影发现乳管内占位性病变的检出率为76.34%(71/93),漏诊率为23.66%(22/93),发现乳管内恶性占位性病变的检出率为20.00%(2/10),漏诊率为80.00%(8/10);超声和X线摄片乳腺导管造影联合,发现乳管内占位性病变的检出率为89.25%(83/93),漏诊率为10.75%(10/93),发现乳管内恶性占位性病变的检出率为60.00%(6/10),漏诊率为40.00%(4/10)。单项检查和联合检查的检出率、漏诊率比较差异均有统计学意义(P<0.05)。结论超声检查和X线摄片乳腺导管造影各有优缺点,两种方法联合检查可提高乳腺导管内占位性病变的检出率,降低漏诊率。
Objective To explore the diagnostic value of galactography and ultrasonography in the treatment of breast intraductal space occupying lesions. Methods From June 2010 to June 2015, 93 patients with breast intraductal space occupying lesions admitted to our hospital and underwent galactography and ultrasonography. The image findings of combined application of two methods were compared with the results of any one of two methods. Results The accuracy rate and misdiagnosis rate of breast intraductal space occupying lesions by ultrasonography were respectively82.80%(77/93) and 17.20%(16/93). The accuracy rate and misdiagnosis rate of breast intraductal malignant space occupying lesions by ultrasonography were respectively 40.00%(4/10) and 60.00%(6/10). The accuracy rate and misdiagnosis rate of breast intraductal space occupying lesions by galactography were respectively 76.34%(71/93) and23.66%(22/93). The accuracy rate and misdiagnosis rate of breast intraductal malignant space occupying lesions by galactography were respectively 20.00%(2/10) and 80.00%(8/10). The accuracy rate and misdiagnosis rate of breast intraductal space occupying lesions by ultrasonography and galactography were respectively 89.25%(83/93) and10.75%(10/93). The accuracy rate and misdiagnosis rate of breast intraductal malignant space occupying lesions by ultrasonography and galactography were respectively 60.00%(6/10) and 40.00%(4/10). There were statistically significant differences of accuracy rate and misdiagnosis rate between combined methods and each single method(P<0.05).Conclusion Ultrasonography and galactography both have their own advantages. Combined application of two methods results in improved accuracy rate and lowered misdiagnosis rate in breast intraductal space occupying lesions.
