摘要
目的分析热休克蛋白27(HSP27)、上皮-钙黏素(E-cadherin)及Ki-67在星型胶质细胞肿瘤中的表达及意义。方法选取WHO分级为Ⅱ级、Ⅲ级、Ⅳ级星型胶质细胞肿瘤标本共40例,采用免疫组化检测HSP27、E-cadherin及Ki-67的表达。结果 HSP27在Ⅱ级胶质瘤的表达(8/13,61.5%)与Ⅲ级(12/13,92.3%),Ⅳ级(14/14,100.0%)比较差异有统计学意义(P<0.05),而Ⅲ级和Ⅳ级的表达比较差异无统计学意义(P>0.05)。87.5%(35/40)的胶质瘤E-cadherin表达出现表达缺失或下降,组间比较差异无统计学意义(P>0.05),但与HSP27的表达之间存在负性趋势。胶质瘤Ki-67指数随WHO分级增高而提高[Ⅱ级:(2.38±1.39)%、Ⅲ级:(11.07±9.73)%、Ⅳ级:(27.31±16.02)%],组间比较差异有统计学意义(P<0.05),并与HSP27的表达正相关(P<0.05)。结论 Hsp27与Ki-67的表达对判断胶质瘤组织学分级及增殖活性具有重要意义。E-cadherin表达下降可能在胶质瘤形成早期即出现,可作为星型胶质细胞发生瘤变的早期判断指标。E-cadherin可能与HSP27共同参与胶质瘤的演进。
Objective To discuss the expression and significances of heat-shock protein 27(HSP27), E-cadherin and Ki-67 in astrocytic tumor. Methods Forty samples of astrocytic tumor were diagnosed by the Department of Pathology of Haikou People’s Hospital and divided into diffusely astrocytoma(WHO grade Ⅱ, 13 cases), astrocytoma(WHO grade Ⅲ, 13 cases) and glioblastoma multiforme(WHO grade Ⅳ, 14 cases) according to WHO clasification of tumours of the central nervous system. Expression of HSP27, E-cadherin and Ki-67 was detected by immunohistochemistry. Results The positive expression of HSP27 in the grade Ⅱ(8/13, 61.5%) and grade Ⅲ(12/13, 92.3%), grade Ⅳgliomas(14/14, 100.0%) were significantly different(P<0.05). However, there was no significant difference in the expression of HSP27 between grade Ⅲ and Ⅳ astrocytoma(P>0.05). The rate of E-cadherin expression missing or decreased were 87.5%(35/40) in astrocytoma, and there was no significant difference among the three groups(P>0.05). E-cadherin expression showed negative trend with the expression of HSP27. The Ki-67 index of astrocytoma increased with the increase of WHO grade [grade Ⅱ:(238±1.39)%, grade Ⅲ:(11.07±973)%, grade Ⅳ:(27.31±1.602)%], and the difference between the groups has statistical significance(P<0.05). Ki-67 index was positively correlated with the expression of HSP27(P<0.05). Conclusion Expression of HSP27 and Ki-67 is meaningful to evaluate tumor pathologic grading and associated with the biological behavior of astrocytic tumor. Abnormal expression of E-cadherin maybe an early event in the process of tumorigenesis, which can be regarded as an index for early diagnosis. The low expression of E-cadherin and over expression of hsp27 may play an important role in the development of astrocytic tumor.
