腹泻儿童血清锌水平与腹泻病程及免疫水平的相关性研究 被引量:15

摘要 目的探讨腹泻儿童血清锌水平与腹泻病程、免疫水平的相关性,为有效控制儿童腹泻提供理论依据。方法将2012年2月至2014年8月期间在我院儿科住院接受治疗的330例腹泻儿童纳入研究。通过医院HIS系统查询400例健康儿童查体数据,比较腹泻儿童与健康儿童血清锌和免疫蛋白含量。将330例患者按照随机数字表分为观察组164例和对照组166例,分别接受常规治疗联合锌制剂疗法和单纯常规疗法治疗2周,随访时间至少1个月,观察两组患者治疗前后血清锌、免疫蛋白含量、粪便性状复常时间、有效率、复发率等指标。结果健康儿童血锌为(101.75±9.33)μmol/L,明显高于腹泻儿童的(50.19±1.77)μmol/L,差异具有显著统计学意义(P<0.01);免疫蛋白含量方面,健康儿童IgA、IgM和IgG均明显高于腹泻儿童,差异均具有统计学意义(P<0.01);对腹泻儿童进行治疗后,发现观察组儿童的血锌为(96.75±8.59)μmol/L,明显高于对照组的(71.55±9.48)μmol/L,差异具有统计学意义(P<0.05);免疫蛋白方面,观察组儿童治疗后的IgA、IgM和IgG均高于对照组,差异均具有统计学意义(P<0.05);观察组儿童粪便性状恢复正常时间为(6.8±2.1)d,明显短于对照组的(10.14±1.9)d,差异具有统计学意义(P<0.05);观察组的治疗有效率和复发率分别为96%和6%,明显优于对照组的71%和14%,两组比较差异均具有统计学意义(P<0.05)。结论儿童血清锌水平与其腹泻病程和免疫水平密切相关,锌制剂联合常规疗法治疗儿童腹泻安全有效,值得临床推广应用。 Objective To investigate the correlation between serum zinc level and the duration of diarrhea,immune level in children with diarrhea, and provide the theoretical basis for effective control of diarrhea in children.Methods From February 2012 to August 2014, 330 children with diarrhea were enrolled in the Department of Pediatrics in our hospital. The serum levels of zinc and immune protein in children with diarrhea and 400 healthy children from the HIS system of the hospital were compared. According to the random number table, 330 patients were divided into the observation group(n=164) and the control group(n=166), who respectively received conventional treatment combined with zinc preparation therapy and simple routine therapy for 2 weeks, and followed up for at least a month to observe serum zinc, immune protein content, excrement recovery time, rate and recurrence rate index of the two groups of patients before and after treatment. Results Blood zinc of healthy children [(101.75+9.33) μmol/L] was significantly higher than that of diarrhea children [(50.19±1.77) μmol/L](P<0.01); As for immune protein content, IgA,IgM and IgG of healthy children were significantly higher than those of children with diarrhea(P<0.01). After the treatment, the blood zinc of observation group [(96.75 ± 8.59) μ mol/L] was significantly higher than that of the control group [(71.55±9.48) μmol/L](P<0.05); The immune protein IgA, IgM and IgG in the observation group were significantly higher than those in the control group(P<0.05); The time of stool back to normal in the observation group[(6.8±2.1) days] was significantly shorter than that in the control group [(10.14±1.9) days](P<0.05); The treatment effective rate and recurrence rate of the observation group(96%, 6%, respectively) were significantly better than the control group(71%, 14%, respectively), P<0.05. Conclusion The duration of diarrhea and immune level are closely related to the level of serum zinc in children with diarrhea. Zinc preparation combined with conventional therapy is safe and effective in the treatment of diarrhea in children, which is worthy of clinical application.
