摘要 目的调查海口地区妇女高危型人乳头瘤病毒(HPV)感染及分布情况,为今后宫颈癌及癌前病变筛查及防治提供依据。方法收集2013年1月至2015年9月在我院就诊的10 840例妇女的宫颈脱落细胞,运用Cervista酶切信号放大检测实验行高危型HPV检测及分型组(依据病毒DNA序列相近性分为A5/A6、A7、A9组)。依据年龄分为青年组(20~29岁),中青年组(30~39岁),中年组(40~49岁),中老年组(50~59岁)和老年组(≥60岁)。统计分析高危型HPV感染与各年龄的关系及各型组分布情况。结果 10 840例标本中共检出高危型HPV阳性者2 010例,感染总阳性率为18.54%,以老年组患者感染率最高(27.61%),其次为中老年组(22.00%),以中青年组感染率最低(16.87%),差异有显著统计学意义(P<0.01)。A9组(包括单重及双重)感染率最高(9.65%),A7组(包括单重及双重)感染率最低(4.65%),差异有显著统计学意义(P<0.01)。结论海口地区妇女高危型HPV感染率具有随年龄增加而增加的趋势,以A9组感染率最高,对今后HPV疫苗研究及早期筛查治疗提供依据。 Objective To investigate the infection status and distribution situation of high-risk(HR) humanpapilloma vires(HPV) in women in Haikou to provide evidence for screening and prevention of cervical cancer and precancerous lesions in the future. Methods A total of 10 840 women of cervical exfoliated cells from January 2013 to September 2015 in our hospital were collected. Cervista HPV HR test was used to detect high-risk HPV and typing(classified into A5/A6, A7 and A9 group based on similarity of the virus DNA sequence). The patients were divided into young group(20 to 29 years old), middle-young-aged group(30 to 39 years old), middle-aged group(40 to 49 years old), middle-elderly-aged group(50 to 59 years old) and elderly group(60 years old or older) on the basis of age. The relationship between high-risk HPV infection and age, and the distribution of each group were statistically analyzed.Results A total of 2 010 positive cases of high-risk HPV were detected in 10 840 cases, and the total positive rate was18.54%. The infection rate of elderly group was the highest(27.61%), followed by middle-elderly-aged group(22.00%),and the infection rate of middle-young-aged group was the lowest(16.87%), with statistically significant differences(P<0.01). The infection rate of A9 group(both single and dual) was the highest(9.65%), and the infection rate of A7 group(both single and dual) was the lowest(4.65%), with statistically significant differences(P<0.01). Conclusion The infection rate of high-risk HPV of women in Haikou area has a increasing trend with age, and the infection rate of A9 group is the highest, which provides a basis for HPV vaccine research and early screening and treatment of high-risk HPV.
