常规超声联合超声弹性成像在乳腺癌术后腋窝淋巴结性质诊断中的应用 被引量:21

摘要 目的探讨常规超声联合超声弹性成像在乳腺癌术后腋窝淋巴结性质诊断中的价值。方法选择我院2012年1月至2015年3月收治的81例接受手术治疗的乳腺癌患者,采用常规超声联合超声弹性成像检查同侧腋窝肿大淋巴结,以术后病理检查或超声引导下穿刺活检结果为金标准,观察常规超声、超声弹性成像及两者联合的诊断价值。结果常规超声诊断腋窝淋巴结性质敏感性和特异性为72.55%和65.71%,超声弹性成像为82.35%、62.86%;常规超声联合超声弹性成像为90.20%、77.14%;常规超声联合超声弹性成像敏感性和特异性均高于常规超声、超声弹性成像单独诊断,差异均有统计学意义(P<0.05)。结论常规超声联合超声弹性成像可提高乳腺癌术后腋窝淋巴结性质诊断的敏感性和特异性,具有较高的临床应用价值。 Objective To investigate the value of conventional ultrasound combined with ultrasonic elasticity imaging in diagnosis of axillary lymph nodes after radical mastectomy for breast cancer. Methods Eighty-one patients with breast cancer who received surgical treatment in our hospital from January 2012 to March 2015 were selected. Conventional ultrasound combined with ultrasonic elasticity imaging was used to examine ipsilateral axillary lymph nodes.The gold standard was the results of postoperative pathological examination or ultrasound-guided puncture biopsy. The diagnostic value of conventional ultrasound, ultrasound elasticity imaging and conventional ultrasound combined with ultrasonic elasticity imaging were observed. Results The sensitivity and specificity in diagnosis of axillary lymph nodes were 72.55% and 65.71% for conventional ultrasound and 82.35%, 62.86% for ultrasonic elasticity imaging, which were significantly lower than those for conventional ultrasound combined with ultrasonic elasticity imaging of 90.20%,77.14%(P<0.05). Conclusion Conventional ultrasound combined with ultrasonic elasticity imaging can improve the sensitivity and specificity in the diagnosis of axillary lymph nodes after radical mastectomy for breast cancer, which has high clinical application value.
