大黄利胆胶囊对胆囊结石患者胆汁成分的影响

Influence of Rhubarb choleretic capsule on the composition of gallbladder bile in patients with gallbladder calculi

目的探讨大黄利胆胶囊对胆囊结石患者胆汁成分的影响。方法选取河北北方学院附属第二医院2012年12月至2014年11月收治的择期行腹腔镜胆囊结石手术(LC)患者160例,按数表法随机分为观察组与对照组各80例,观察组患者术前两周给予大黄利胆胶囊口服,对照组术前不服用任何药物,观察两组患者胆囊切除术后一般状况,检测两组患者胆汁中结合胆红素、总胆红素、胆固醇和胆汁酸的含量,计算出未结合胆红素(即游离胆红素)占总胆红素百分比(UCB%)。以胆固醇含量(TC)、胆汁酸含量(TBA)和USB%为指标,比较两组之间的差异。结果两组患者术后一般情况比较差异均无统计学意义(P>0.05);观察组患者胆汁中TC、USB%水平分别为(16.14±3.90)mmol/L、(0.139±0.090)mmol/L,对照组分别为(17.66±3.78)mmol/L、(0.187±0.149)mmol/L,观察组TC、USB%明显低于对照组,差异均具有统计学意义(P<0.05);观察组患者胆汁中TBA水平为(119.1±13.5)mmol/L,明显高于对照组的(106.7±10.2)mmol/L,差异具有统计学意义(P<0.05)。结论大黄利胆胶囊可影响胆囊结石患者胆汁成分,对胆囊结石具有良好的治疗和预防作用。

Objective To explore the influence of Rhubarb choleretic capsule on the composition of gallbladder bile in patients with gallbladder calculi. Methods A total of one hundred and sixty patients with gallbladder calculi,who admitted to our hospital and underwent cholecystectomy from December 2012 to November 2014, were randomly divided into the treatment group and the control group, with 80 cases in each group. The patients in treatment group were given Rhubarb choleretic capsule in preoperative, and the patients in control group were given nothing. The general conditions of the patients was observed. Conjugated bilirubin, total bilirubin, cholesterol and bile acid were measured for each sample,and the percentage of unconjugated bilirubin(UCB%) was counted. The total cholesterol(TC), total bile acid concentration(TBA) and UCB% of the two groups were calculated and compared. Results The postoperative general situation of the two groups showed no statistically significant difference(P>0.05). The TC and USB% level in the treatment group were(16.14±3.90) mmol/L and(0.139±0.090) mmol/L, which were significantly lower than(17.66±3.78) mmol/L and(0.187±0.149) mmol/L of the control group(P<0.05). TBA of the treatment group was(119.1±13.5) mmol/L, which was significantly higher than(106.7±10.2) mmol/L of the control group(P<0.05). Conclusion Rhubarb choleretic capsule can impact the composition of gallbladder bile, and has good treatment and prevention effect on gallstones.
