腹针联合解结速刺针法治疗气滞血瘀型月经不调的临床研究

Effect of abdominal acupuncture combined with resolving knot and fast needling in the treatment of Qi stagnation and blood stasis type irregular menstruation

目的观察腹针联合解结速刺法对气滞血瘀型月经不调患者的性激素及临床证候的影响并评价疗效。方法选取广州医科大学附属第三医院针灸门诊2016年10月至2017年10月间收治的气滞血瘀型月经不调患者70例作为研究对象,根据随机数表法将其分为观察组和对照组各35例,观察组采用腹针联合解结速刺针法治疗,对照组采用单纯腹针治疗,两组患者均为月经干净后,1个月为一个周期,治疗两个周期后,评价两组患者治疗前后的激素水平变化、证候积分变化并对比治疗有效率。结果治疗后,观察组患者的血清卵泡雌激素(FSH)、黄体生成素(LH)、催乳素(PRL)分别为(7.07±2.06) IU/L、(8.18±2.03) IU/L、(35.21±3.59)μg/L,明显低于对照组的(9.38±2.26) IU/L、(10.25±2.76) IU/L、(32.03±4.24)μg/L,而观察组患者的血清雌二醇(E2)、孕酮(PROG)分别为(112.84±28.21) pmol/L、(0.36±0.10)μg/L,明显高于对照组的(92.55±23.72) pmol/L、(0.31±0.07)μg/L,差异均有统计学意义(P<0.05);治疗后观察组患者的证候积分为(4.94±1.28)分,明显低于对照组的(9.54±3.14)分,差异有统计学意义(P<0.05);观察组患者的治疗总有效率为94.3%,明显高于对照组的74.3%,差异有统计学意义(P<0.05)。结论腹针联合解结速刺针法能有效改善月经不调患者的激素水平及症状,治疗效果显著,值得临床推广。

Objective To evaluate the curative effect of abdominal acupuncture combined with resolving knot and fast needling on hormone levels and clinical symptoms in the patients with Qi stagnation and blood stasis type irregular menstruation.Methods A total of 70 patients with Qi stagnation and blood stasis type irregular menstruation were selected as the research subjects from Department of Acupuncture Clinic,the Third Affiliated Hospital of Guangzhou Medical University from October 2016 to October 2017.According to random number table method,they were divided into the observation group and the control group,with 35 cases in each group.The observation group was treated with abdominal acupuncture combined with resolving knot and fast needling,while the control group was treated with abdominal acupuncture only.The two groups were treated with acupuncture once every other day after menstruation,with one month as a treatment cycle,for 2 cycles.Then the changes of hormone levels and syndrome scores before and after treatment were evaluated,and the efficiency of the treatment was compared.Results After treatment,the serum follicular estrogen(FSH),luteinizing hormone(LH),and prolactin(PRL)in the observation group were(7.07±2.06)IU/L,(8.18±2.03)IU/L,and(35.21±3.59)μg/L,respectively,significantly lower than(9.38±2.26)IU/L,(10.25±2.76)IU/L,(32.03±4.24)μg/L in the control group.The serum estradiol(E2),and progesterone(PROG)of the observation group were(112.84±28.21)pmol/L,and(0.36±0.10)μg/L,respectively,significantly higher than(92.55±23.72)pmol/L and(0.31±0.07)μg/L in the control group(P<0.05).After treatment,the syndrome score of the observation group was 4.94±1.28,which was significantly lower than 9.54±3.14 in the control group(P<0.05).The total effective rate of the observation group(94.3%)was significantly higher than that of the control group(74.3%),and the difference statistically significant(P<0.05).Conclusion Abdominal acupuncture combined with resolving knot and fast needling is effective in the treatment of Qi stagnation and blood stasis type irregular menstruation.It can effectively regulate hormone levels and symptoms of patients with irregular menstruation,which is worth popularizing.
