应变率成像评价冠心病PCI术治疗前后左室局部功能的变化

Evaluation of left ventricular regional function in patients with coronary heart disease before and after PCI treatment by strain rate imaging

目的探讨采用应变率成像(SRI)评价冠心病经皮冠状动脉介入(PCI)术治疗前后左室局部心肌功能的变化。方法选择2017年2月至2018年2月佛山市南海区第四人民医院心内科诊治的62例行PCI治疗的冠心病患者为研究组,并选择50例门诊体检健康者为对照组,所有受试者均进行超声心动图检查,SRI评价左室局部舒张和收缩功能,研究组冠心病患者于术前、术后1个月进行SRI检查,观察PCI前后左室局部心肌功能的变化。结果研究组患者术前的后间隔中间段、后间隔心尖段、侧壁心尖段、前壁基底段、前壁中间段、前壁心尖段、前间隔基底段、前间隔中间段、前间隔心尖段收缩期应变率(SRs)、舒张早期应变率(SRe)、舒张晚期应变率(SRa)均低于对照组,差异均有统计学意义(P<0.05);治疗后,研究组患者的后间隔中间段、后间隔心尖段、侧壁心尖段、前壁基底段、前壁中间段、前壁心尖段、前间隔基底段、前间隔中间段、前间隔心尖段SRs、SRe、SRa均明显升高,差异均有统计学意义(P<0.05)。结论 PCI术可改善冠心病患者心功能,SRI可定量评价PCI术治疗前后左室局部心肌功能变化。

Objective To evaluate the changes of left ventricular regional myocardial function before and after percutaneous coronary intervention(PCI)in patients with coronary heart disease by strain rate imaging(SRI).Methods From February 2017 to February 2018,62 patients with coronary heart disease treated by PCI in Fourth People’s Hospital of Nanhai District of Foshan City were selected as the study group,and 50 healthy outpatients as the control group.All subjects were examined by echocardiography.The left ventricular regional diastolic and systolic function was evaluated by SRI.Before PCI and 1 month after PCI,SRI examination was performed to observe the changes of left ventricular regional myocardial function.Results The systolic strain rate(SRs),early diastolic strain rate(SRe)and late diastolic strain rate(SRa)of the posterior septal middle segment,posterior septal apex segment,lateral apical segment,anterior basal segment,anterior middle segment,anterior apical segment,anterior septal basal segment,anterior septal middle segment,and anterior apical segment of the study group were lower than those in the control group before operation(P<0.05).The SRs,SRe and SRa of the posterior septal middle segment,posterior septal apex segment,lateral apical segment,anterior basal segment,anterior middle segment,anterior apical segment,anterior septal basal segment,anterior septal middle segment,and anterior apical segment of the study group were significantly increased(P<0.05).Conclusion PCI can improve the cardiac function of patients with coronary heart disease,and SRI can quantitatively evaluate the changes of left ventricular regional myocardial function before and after PCI.
