经空肠减压-肠内营养在合并肺功能不全胃癌根治术中的应用

Application of jejunal decompression and enteral nutrition in radical resection of gastric cancer complicated with pulmonary insufficiency

目的探讨合并呼吸道疾病的胃癌患者术中吻合口减压及肠内营养的疗效。方法回顾性分析2011年7月至2018年3月南通大学附属东台医院收治的12例合并呼吸道疾病胃癌患者的临床资料,所有患者均行根治性全胃切除术,经空肠造口逆行放置胃管于空肠食管吻合口处,顺行放置营养管达空肠下20 cm远处,行吻合口减压及肠内营养,观察其围手术期食管空肠吻合口愈合情况和恢复情况。结果 12例患者均顺利完成手术。围手术期无吻合口瘘,无死亡,恢复良好。结论胃癌根治术中经空肠减压-肠内营养是一种安全、有效的治疗方法,适用于肺功能不全患者。

Objective To evaluate the effect of anastomotic decompression and enteral nutrition in patients with gastric cancer complicated with respiratory diseases.Methods A retrospective analysis was made on 12 patients with gastric cancer complicated with respiratory diseases undergoing radical total gastrectomy at Affiliated Dongtai Hospital of Nantong University from July 2011 to March 2018.Retrograde gastric tube was placed at the jejunoesophageal anastomosis via jejunostomy.The nutritional tube was placed in the distance from the lower jejunum to 20 cm.The anastomotic decompression and enteral nutrition were performed.The scenarios such as perioperative esophageal jejunal anastomosis and postoperative recovery were observed.Results The operation was successfully completed in 12 patients.There were no anastomotic fistula cases and no death cases in perioperative period.Conclusion Jejunal decompression and enteral nutrition in radical gastrectomy is a safe and effective treatment for patients with pulmonary insufficiency.
