SAA、CRP、ESR联合检测在AECOPD病情监测中的价值分析 被引量:11

摘要 目的:探讨联合检测血清淀粉样蛋白A(SAA)、C反应蛋白(CRP)、血沉(ESR)在慢性阻塞性肺疾病急性加重期(AECOPD)病情监测中的临床价值。方法:87例COPD患者分为COPD急性加重期组(n=45例)和COPD稳定期组(n=42例),同期选择40例门诊健康体检者作为对照组。根据是否出现呼吸衰竭、是否合并肺心病将AECOPD患者分为呼吸衰竭组、无呼吸衰竭组和合并肺心病组、未合并肺心病组,比较各组血清SAA、CRP表达水平及ESR变化,并分析血清SAA、CRP及ESR的相关性。结果:COPD急性加重期组患者血清SAA、CRP表达水平及ESR均明显高于COPD稳定期组和对照组(P<0.05);呼吸衰竭组患者血清SAA、CRP表达水平及ESR均明显高于无呼吸衰竭组(P<0.05);合并肺心病组患者血清SAA、CRP表达水平及ESR均明显高于未合并肺心病组(P<0.05),血清SAA表达水平与CRP及ESR均呈正相关性关系(r1=0.576,r2=0.607,P<0.05),血清CRP表达水平与ESR也呈正相关性关系(r3=0.482,P<0.05)。结论:联合检测SAA、CRP、ESR在AECOPD患者病情监测中具有较高的临床应用价值。 Objective:To investigate the clinical value of the combined detection of serum amyloid A(SAA),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)in condition monitoring of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:87patients with COPD were divided into acute exacerbation stage of COPD group(n=45cases)and stable phase of COPD group(n=42cases),and 40 outpatient health examination subjects were selected as the control group.Patients with AECOPD were divided into two subgroups of respiratory failure group/non-respiratory failure group and another two subgroups of complicated with/without pulmonary heart disease group according to respiratory failure and complicated with pulmonary heart disease happen or not.The expression levels ofserum SAA,CRP,ESR of all groups were compared,and the correlation between serum SAA,CRP and ESR was analyzed.Results:The expression levels of serum SAA,CRP and ESR of acute exacerbation stage of COPD group were significantly higher than those of stable phase of COPD group and the control group(P<0.05).The expression levels of serum SAA,CRP and ESR of respiratory failure group were significantly higher than those of non-respiratory failure group(P<0.05).The expression level of serum SAA,CRP and ESR of complicated with pulmonary heart disease group were significantly higher than those of without pulmonary heart disease group(P<0.05).The expression level of SAA was positively correlated relationship with CRP and ESR(r1=0.576,r2=0.607,P<0.05),and the expression levels of serum CRP was positively correlated relationship with ESR(r3=0.482,P<0.05).Conclusion:The combined detection of SAA,CRP and ESR has a high clinical application value of condition monitoring of patients with AECOPD.
