MMP28在膀胱癌中的表达及其临床意义

MMP28 Expression in Blander Cancer and Its Clinical Significance

目的:探讨MMP28在膀胱癌中的表达情况及其与临床分期、组织学分级、预后等指标的关系,分析MMP28表达对膀胱癌预后的影响。方法:收集8对新鲜的膀胱癌组织及其配对的正常膀胱组织和173例膀胱癌组织蜡块,分别采用qRT-PCR和免疫组化法检测MMP28的表达,结合相关病理和随访指标,采用χ2检验、Kaplan-Meier法、Cox回归进行统计分析。结果:在转录水平上,膀胱癌组织的MMP28表达高于正常的膀胱癌组织,有统计学意义。MMP28蛋白在膀胱癌中的表达与其年龄、病理分级、临床分期有关(P=0.039,P=0.024,P=0.003),与性别、肿瘤数量、肿瘤直径无关(P=0.120,P=0.266,P=0.462)。膀胱癌中MMP28蛋白高表达组的总体生存率和无进展生存率显著低于低表达组(P=0.002,P=0.001)。单因素分析发现影响膀胱癌患者预后的因素有患者的年龄(P=0.006,P=0.007)、病理学分级(P=0.008,P=0.006)、肿瘤数目(P=0.012,P=0.013)、临床分期(P=0.003,P=0.003)和MMP28蛋白的表达(P=0.002,P=0.001)。而且,多因素分析表明只有临床分期(P=0.022,P=0.043)、MMP28(P=0.037,P=0.045)是预测膀胱癌总体生存和无进展生存的独立因素。结论:MMP28在膀胱癌中的表达是明显上调的。MMP28表达与膀胱癌的年龄较大、高组织分级、较晚临床分期、预后差呈正相关,是预测膀胱癌生存的一个独立因素,可能成为膀胱癌治疗的一个新靶点。

Objective:To investigate the effect of MMP28 on bladder cancer clinicopathologic characteristics and progression.Methods:Expression of MMP28 in 8frozen bladder cancer tissues and paired normal bladder tissues were dectected by real-time polymerase.And expression of MMP28 in 173paraffin-embedded stored specimens from patients with bladder cancer were dectected by immunohistochemical stain in this study.Statistical analysis of the relationship between the expression of MMP28 and clinicopathologic characteristics or progression was performed byχ2 test,Kaplan-Meier method and cox regression.Results:Compared with paired normal baldder tissue,expression of MMP28 RNA was significantly increased in bladder cancer tissue.Immunochemistry data indicated expression of MMP28 protein was closely related to age,grade,and clinical stage(P=0.039,P=0.024,and P=0.003,respectively),rather than its gender,and tumorsize,and tumor number(P=0.120,P=0.266,and P=0.462,respectively).Survival analysis suggest expression of MMP28 in bladder cancer had a significant relation to overall survival rate and progression-free survival rate(P=0.002,P=0.001).Univariate analysis indicated the prognosis of bladder cancer was associated to age(P=0.006,P=0.007),grade(P=0.008,P=0.006),tumor number(P=0.012,P=0.013),clinical stage(P=0.003,P=0.003),and MMP28(P =0.002,P =0.001).Moreover,Multivariate analysis suggested expression of MMP28(P=0.037,P=0.045)was the only independent progonostic factor in bladder cancer besides clincal stage(P=0.022,P=0.043).Conclusion:The expression of MMP28 in bladdder cancer was significantly increased.The expression of MMP28 in bladdder cancer has a positive correlation with older age,high histological grade,late clinical stage and poor prognosis.As a independent progonostic factor in bladder cancer,MMP28 can be a potential target for the treatment.
