超声引导下细针穿刺甲状腺微小结节针感与超声造影相关性分析 被引量:6

Relationship Between Contrast-Enhanced Ultrasonography and FNA Needle Feeling in the Differential Diagnosis of Thyroid Micro Nodules

摘要 目的:探讨TI-RADS 3-4级甲状腺微小结节超声造影(CEUS)增强模式与超声引导下细针穿刺(FNA)结节质地的相关性。方法:通过对149例TI-RADS 3-4级甲状腺微小结节进行CEUS检查记录增强模式并行FNA,记录穿刺针感,比较良恶性结节增强模式与针感的关系。结果:良性组中:等/高增强占60.9%(53/87),其中有针感的占3.8%(2/53),无针感的占96.2%(51/53);低增强占39.1%(34/87),其中有针感占35.3%(12/34),无针感占64.7%(22/34);Spearman相关系数为0.386,P<0.01。恶性组中:等/高增强占35.5%(22/62),其中有针感占59.1%(13/22),无针感占40.9%(9/22);低增强占64.5%(40/62),其中有针感的92.5%(37/40),无针感的7.5%(3/40);Spearman相关系数为0.415,P<0.01。结论:CEUS及FNA针感在鉴别TI-RADS 3-4级微小结节的诊断中具有一定相关性。 Objective:To analyze the correlation between contrast-enhanced ultrasonography(CEUS)enhancement pattern and FNA needle feeling in the differential diagnosis of TI-RADS 3-4 thyroid small nodule.Methods:Through CEUS examination of 149 cases of TI-RADS grade 3-4 nodules,we recorded the enhancement pattern parallel FNA,recorded the puncture needle feeling,and compared the relationship between benign and malignant nodules enhancement pattern and FNA needle feeling.Results:Of the 149 thyroid nodules,87 were benign and 62 malignant.In the benign group,the equal and high enhancement accounted for 60.9%,of which there were 2 cases of needling feeling and 51 cases of needle free feeling.The low enhancement rate was 39.1%,of which there were 12 cases of needling feeling and 22 cases of needle free feeling.The correlation coefficient of Spearman was 0.386,P<0.01.In the malignant group,the equal and high enhancement accounted for 35.5%,of which there were 13 cases of needling feeling and 9 cases of needle free feeling;the low enhancement accounted for 64.5%,of which there were 37 cases of needling feeling and 3 cases of needle free feeling.Conclusion:CEUS and FNA sensitivity in thedifferential diagnosis of TI-RADS grade 3-4 nodules may have significant correlation.
