种植修复在老年患者牙列游离端缺失中的应用

Application of Implant Repair in Free End Loss of Dentition in Elderly Patients

目的:探讨种植牙技术在老年牙列游离端缺失患者患牙咬合效率及不良反应发生率的影响。方法:选取2012年1月-2016年9月72例老年牙列游离端缺失患者,随机数字表法分为对照组(n=36)与研究组(n=36)。对照组采用常规可摘局部义齿修复治疗,研究组采用种植牙技术进行修复治疗。术后6个月统计对比两组治疗效果(种植成功率、咬合效率)、治疗满意度、入院时及术后1个月生活质量评分(QOL)变化情况及不良反应发生率。结果:研究组种植成功率94.44%(34/36)、咬合效率91.67%(33/36)高于对照组[72.22%(26/36)、69.44%(25/36)](P<0.05);两组治疗总满意度比较,研究组94.44%(34/36)高于对照组69.44%(25/36)(P<0.05);治疗后两组QOL评分较治疗前增高,且研究组高于对照组(P<0.05);两组不良反应发生率比较,研究组8.34%(3/36)低于对照组27.29%(10/36)(P<0.05)。结论:采用种植牙技术治疗老年牙列游离端缺失患者效果显著,种植成功率高,且咬合效率较好,可有效提高患者治疗满意度,改善其生活质量,且不良反应发生率低,安全性高,具有推广价值。

Objective:To explore the effect of dental implant technique on the occlusal efficiency and the incidence of adverse reactions in the elderly patients with dentition free end missing.Methods:From January 2012 to September 2016,72 elderly patients with dentition free end missing were randomly divided into control group(n=36)and dental implant group(n=36).The control group was treated with conventional removable partial denture,and the dental implant group was conducted with implant dental technique.The treatment effect(success rate,occlusal efficiency),treatment satisfaction,quality of life(QOL)at admission and 1 month after surgery,and the incidence of adverse reactions were statistically compared between the two groups 6 months after operation.Results:The success rate of the treatment in dental implant group was 94.44%(34/36),the occlusal efficiency was 91.67%(33/36),which were higher than those in the control group(72.22% [26/36]and 69.44%[25/36],respectively),the difference was statistically significant(P<0.05);The overall satisfaction of dental implant group with 94.44%(34/36)was higher than the control group with 69.44%(25/36)(P<0.05);After treatment,the scores ofQOL in the two groups were higher than those before treatment,and dental implant group was higher than the control group(P<0.05);The incidence of adverse reactions in dental implant group(8.34%,3/36)was significantly lower than that in the control group(27.29%,10/36)(P<0.05).Conclusion:The effect of dental implant technique in the treatment of elderly patients with dentition free end missing is significant,the success rate of treatment is high,and the occlusion efficiency is better,it also can increase the patient’s satisfaction,improve the quality of life and decrease the incidence of adverse reactions with high security.
