种植体周围病的发生率及其危险因素 被引量:2

The incidence and risk factors of peri-implant diseases

摘要 目的:探讨种植治疗患者周围黏膜炎及种植体周围炎的发生情况及危险因素。方法:对我院完成的167例种植治疗的临床资料进行分析,以炎症发生情况分为黏膜炎组、周围炎组、健康组,比较3组一般资料及治疗后菌斑牙石指数(PLI)、基质金属蛋白酶(MMP)、基质金属蛋白酶抑制剂(TIMP),使用Logistic回归分析周围黏膜炎及种植体周围炎的危险因素。结果:周围黏膜炎及种植体周围炎发生率分别为71. 26%、10. 78%。周围炎组MMP-9、TIMP-1显著高于黏膜炎组及健康组(P<0. 05),黏膜炎组上述指标显著高于健康组(P<0. 05);Logistic回归分析显示,吸烟、术前探查深度≥6 mm、PLI≥2、MMP-9、TIMP-1是周围黏膜炎及种植体周围炎的独立危险因素。结论:目前种植体周围病发病率较高,吸烟、术前探查深度≥6 mm、PLI≥2、术后MMP-9及TIMP-1是植体周围病的独立危险因素。 Objective:To investigate the occurrence of peri-implant mucositis and peri-implantitis and their risk factors.Methods:A retrospective analysis was performed on 167 implant patients in our hospital,the patients were divided into peri-implant mucositis group(167 cases),peri-implantitis group(18 cases)and healthy group(30 cases)according to different inflammation.The general data,plaque calculus Index(PLI),Metalloproteinase(MMP)and Tissue Inhibitor of Metalloproteinase(TIMP)of three groups were compared,Logistic regression was used to analyze the risk factors of peri-implant mucositis and peri-implantitis.Results:The incidence of peri-implant mucositis and peri-implantitis were71.26%and 10.78%,respectively.The MMP and TIMP in peri-implantitis group were significantly higher than in peri-implant mucositis group and healthy group(P<0.05),and the MMP and TIMP in peri-implant mucositis group were significantly higher than in healthy group(P<0.05).Multivariate Logistic regression analysis showed that smoking,presence of pocket depth≥6 mm,plaque calculus Index≥2,MMP,and TIMP were independent risk factors for peri-implant mucositis and peri-implantitis.Conclusion:Peri-implant diseases are popular in patients with implantation,and smoking,presence of pocket depths≥6 mm,plaque calculus Index≥2,MMP-9,and TIMP-1 are independent risk factors for peri-implant diseases.
