磁共振成像对胆管癌的诊断价值

The Value of Magnetic Resonance Imaging in the Diagnosis of Bile Duct Carcinoma

目的探讨在胆管癌诊断中应用磁共振成像的临床价值。方法随机选取2016年2月至2018年2月本院收治的胆管癌患者42例作为研究对象,另选择同时间段本院收治的肝局灶性占位病变患者42例和健康体检者42例作为研究对象,均行磁共振成像检查,并测量表观扩散系数,探讨磁共振成像诊断胆管癌诊的影像学特点。结果胆管癌DWI为信号混杂,衰减低于肝实质,病灶呈高信号。健康体检者、肝血管瘤患者、肝囊肿患者ADC值显著高于胆管癌患者,具有统计学意义(P<0.05);而胆管癌患者与肝细胞癌患者、肝转移瘤患者ADC值则差异无统计学意义(P>0.05);肝良性病变患者ADC值最高,且显著高于肝恶性病变患者和肝管癌患者,差异有统计学意义(P<0.05)。结论在胆管癌诊断中应用磁共振成像具有较高的价值,可有效鉴别肝局灶性占位病变与胆管癌,为疾病的治疗和控制提供可靠依据,值得推广应用。

Objective To evaluate the clinical value of magnetic resonance imaging in the diagnosis of cholangiocarcinoma.Methods From February 2016 to February 2018,42 patients with cholangiocarcinoma,42 patients with focal occupying lesions of liver and 42 healthy controls were selected randomly.All patients were examined by magnetic resonance imaging(MRI)and the diffusion coefficient was measured to investigate the imaging features of MRI in diagnosis of cholangiocarcinoma.Results The DWI signal of cholangiocarcinoma was mixed and the attenuation was lower than that of hepatic parenchyma.Healthy person,liver heman gioma,liver cyst The ADC value in patients with cholangiocarcinoma was significantly higher than that in patients with cholangiocarcinoma(P<0.05),but the ADC value in patients with cholangiocarcinoma and hepatocellular carcinoma had no significant difference(P>0.05).It was significantly lower than that in patients with malignant liver disease and hepatic duct cancer(P<0.05).Conclusion Magnetic resonance imaging has a high value in the diagnosis of cholangiocarcinoma.It can effectively distinguish focal occupying lesions from cholangiocarcinoma and provide reliable basis for the treatment and control of the disease.It is worth popularizing.
