摘要
目的比较分析1.5T核磁共振(1.5T MRI)、64排螺旋CT(64-MSCT)扫描检查诊断膝关节隐匿性骨折的价值。方法选取2016年2月至2018年2月我院收治的61例膝关节隐匿性骨折患者,分别行1.5T MRI扫描、64-MSCT检查,记录1.5T MRI与64-MSCT影像学征象,并统计1.5T MRI与64-MSCT诊断阳性率。结果 61例膝关节隐匿性骨折患者:1.5T MRI检查共检出骨折91处,所有病灶T2W1均可在T1WI低信号周围见片状高信号区域,部分病灶T1WI低信号周围镶嵌高信号区域,对应层面T2W1表现为高信号;64-MSCT检查共检出32例膝关节隐匿性骨折40处,图像可见骨小梁扭曲,连续性中断或骨皮质断裂;1.5T MRI诊断膝关节隐匿性骨折阳性率高于64-MSCT,且骨髓水肿、半月板软骨损伤、关节腔积液、韧带损伤、关节面软骨破损与水肿检出率高于64-MSCT(P<0.05)。结论与64-MSCT检查相比,1.5T MRI能清晰显示关节囊内充血、关节腔内积液,诊断膝关节隐匿性骨折阳性率较高,值得推广。
Objective To compare the value of 1.5 T MRI and 64-slice spiral CT(64-MSCT)scan in the diagnosis of occult fractures of the knee.Methods Sixty-one patients with occult fractures of the knee joint admitted to our hospital from February 2016 to February 2018 were enrolled in the study.1.5 T MRI scan and 64-MSCT were performed to record 1.5 T MRI and 64-MSCT imaging findings and 1.5 T MRI and 64-MSCT diagnostic positive rate.Results Sixty-one patients with occult fractures of the knee:91 fractures were detected by 1.5 T MRI.All lesions T2 W1 could be seen in the high signal area around the T1 WI low signal.Some lesions were embedded with high signal areas around the T1 WI low signal.Level T2 W1 showed high signal;40 occult fracturesin 32 cases of were detected by 64-MSCT examination.The images showed trabecular bone distortion,continuous interruption or cortical bone fracture.1.5 T MRI diagnosis of knee occult fracture of positive rate is above the 64-MSCT detection rate.The detection rate of bone marrow edema,meniscus cartilage injury,joint cavity effusion,ligament injury,articular cartilage damage and edema was higher than 64-MSCT(P<0.05).Conclusion Compared with the 64-MSCT examination,1.5 T MRI can clearly show the hyperemia of the joint capsule,the effusion in the joint cavity,and the high positive rate of the occult fracture of the knee joint,which is worthy of promotion.
