宫颈扩张球囊联合间苯三酚对瘢痕子宫晚期妊娠孕妇宫颈成熟效果及妊娠结局的影响

Effects of Cervical Dilation Balloon Combinedwith Phloroglucinol on Cervical Maturation and Pregnancy Outcome of ScarredUterine Pregnant Women in Late Pregnancy

目的探讨宫颈扩张球囊联合间苯三酚对瘢痕子宫晚期妊娠孕妇宫颈成熟效果及妊娠结局的影响。方法选取2018年1月至12月佛山市顺德区妇幼保健院和高州市妇幼保健院产科收治的瘢痕子宫晚期妊娠孕妇35例,按数字奇偶法分为对照组18例和观察组17例,对照组给予宫颈扩张球囊,观察组在对照组基础上联合间苯三酚引产,比较两组促宫颈成熟效果,详细记录两组产程、分娩方式及妊娠结局。结果观察组促宫颈成熟总有效率高于对照组(P<0.05);观察组临产时间、第一产程、第三产程、总产程时间均短于对照组(P<0.05),两组第二产程比较差异无统计学意义(P>0.05);观察组剖宫产率低于对照组,且不良妊娠结局发生率低于对照组(P<0.05)。结论在宫颈扩张球囊基础上联合间苯三酚引产可有效促进瘢痕子宫晚期妊娠孕妇宫颈成熟,缩短产程,降低剖宫产率,有利于改善妊娠结局。

Objective To investigate the effects of cervical dilation balloon combined with phloroglucinol on cervical maturation and pregnancy outcome of scarred uterine pregnant women in late pregnancy.Methods 35 scarred uterine pregnant women in late pregnancyadmittedto Shunde Maternal and Child Health-Care Hospital of Foshan City and obstetrics department of Gaozhou Maternal and Child Health-Care Hospital from January 2018 to December 2018,which were divided into control group(18 cases)and observation group(17 cases)according to digital parity method.The control group was given cervical dilatation balloon,while the observation group was induced labor by phloroglucinol on the basis of the control group.The effect of promoting cervical maturation was compared between the two groups.The birth process,delivery mode and pregnancy outcome of the two groups were detailedrecorded.Results The effective rate ofcervicalmaturationin the observation group was higher than that in the control group(P<0.05).The labor time,the first stage,the third stage,and the total labor time in the observation group were all shorter than those in the control group(P<0.05).There was no significant difference in the second stage of laborbetween the two groups(P>0.05).The rate of cesarean section in the observation group was lower than that in the control group,and the incidence of adverse pregnancy outcome was lower than that in the control group(P<0.05).Conclusion Induction of labor with phloroglucinol on the basis of cervical dilation balloon can effectively promote the cervical maturation of scarred uterine pregnantin late pregnancy,shorten the labor process and reduce the rate of cesarean section,which is beneficial to improve the pregnancy outcome.
