肾病综合征并发下肢静脉血栓的预防及治疗探讨

Prevention and Treatment of Lower Limb Venous Thrombosis Accompanied by Nephrotic Syndrome

目的 探讨对肾病综合征合并下肢静脉血栓的预防及治疗效果。方法 1 1 4例肾病综合征 (NS)患者 ,A组 32例用华法令抗凝 ,B组 31例用潘生丁抗凝 ,C组 51例未用抗凝剂 ,分别观察 3组病人下肢静脉血栓的发生情况。出现下肢静脉血栓患者用尿激酶 (UK) 2 0 0 0 0 0U每日局部静脉滴注 ,治疗 7～ 1 4天。结果 发生下肢静脉血栓在A组未见 ,在B组有 2例 ,在C组有 1 5例。C组分别与A、B两组比较 ,均为P <0 0 1 ,而A组与B组比较P >0 0 5。1 7例下肢静脉血栓用尿激酶治疗后均有下肢浮肿消退或减轻。结论 华法令和潘生丁都有预防肾病综合征并发下肢静脉血栓的作用 ,而华法令可能比潘生丁预防作用更强 ;

Objective To investigate the effect of prevention and treatment of lower limb venous thrombosis accompanied by nephrotic syndrome. Methods 114 patients with nephrotic syndrome were divided into three groups.Warfarin was taken orally in 32 patients of group A ;Agacore was given in 31 patients of group B; No anticoagulant therapy in 51 patients of group C. 200000 U/d of urokinase was given into local vein 7～14 days when patients with lower limb venous thrombosis. Results 2 cases of group B suffered from lower limb venous thrombosis, 15 cases of group C with it ,and none of group A with it.Group A vs group B P >0 05,group A vs group C P< 0 05 and group B vs group C P <0 05.Edema of every patient accompanied with lower limb venous thrombosis declined after treatment with Urokinase.Conclusion Warfarin and agacore may be prevented from lower limb thrombosis with nephrotic syndrome.Warfarin has stronger effect. Urokinase given into local vein may take obvious effect when a patient with nephrotic syndrome suffered from lower limb venous thrombosis.
