摘要
目的 :探讨肾移植术后巨细胞病毒 (CMV)肺炎的治疗措施。方法 :对 8例肾移植术后发生巨细胞病毒肺炎的临床资料进行回顾分析。结果 :本组 6例患者均于发热后 1～ 2天入院 ,并给予以更昔洛韦为主的综合治疗 ,全部治愈出院。另外 2例于入院前已发热 10～ 15天 ,并已作常规抗炎治疗无效而入院 ,入院后迅速发展为呼吸窘迫综合征 (ARDS) ,最后死于呼吸衰竭。结论 :对肾移植术后巨细胞病毒肺炎应尽早诊断、尽早用药 ,以更昔洛韦或膦甲酸钠为主的综合治疗 (包括调整免疫抑制剂 ,纠正低蛋白血症支持等治疗 )为佳。
Objective:To explore the therapy of cytomegalovirus pneumonia after kidney transplantation Methods:Clinical data of 8 patients undergoing cytomegalovirus pneumonia after kidney transplantation were retrospectively analyzed Results:6 cases were treated immediately with Ganciclovir and recoveried ultimately,2 cases with fever delayed for 10 to 15 days because of ineffective anti inflammation died from acute respiratory distress syndrome Conclusion:Cytomegalovirus pneumonia after kidney transplantation should be diagnosed and treated with ganciclovir of forscarnet immediately,the general therapy should be adjusted including the dosage of immunosuppressor and the use of sustaining therapy such as immunoglobulin will minimize the mortality rate
