脊髓髓内生殖细胞瘤1例及文献复习

Intramedullary germinoma of spinal cord: a case report and review of literature

目的 分析脊髓髓内生殖细胞瘤临床病理学特征。方法 报告 1例罕见的伴有滋养层细胞成分的脊髓髓内生殖细胞瘤 ,同时复习相关文献。结果 脊髓髓内生殖细胞瘤的组织学改变类似于性腺原基发生的生殖细胞瘤。免疫组化染色肿瘤细胞PLAP(+)、AFP(- ) ,当伴有滋养层细胞时 β HCG(+)。 结论 脊髓髓内生殖细胞瘤是一种罕见的肿瘤 ,应该与一些脊髓发生的肿瘤相鉴别。该瘤对放射治疗极为敏感。

Objective To analyze the clinico pathological features of intramedullary germinoma in spinal cord Methods A case of intramedullary germinoma in spinal cord with syncytiotrophoblastic giant cells (STGCs) was described and related literature was reviewed Results The histological picture of intramedullary germinoma was similar to that of the gonadal counterparts Immunohistochemical staining for PLAP was positive, and AFP negative in tumor cells When there were STGCs in the specimen, STGCs was immunoreactive for β HCG Conclusions Germinoma with STGCs is a rare tumor in spinal cord It should be differentiated from intramedullary spinal cord tumors including ependmoma, chordoma and metastatic tumors Germinoma is highly radiosensitive
