经尿道电气化术治疗前列腺增生的远期疗效

Evaluation of the long-term outcome about transurethral vaporization of prostate for benign prostatic hyperplasia.

目的 :评价经尿道电气化术 (TUVP)治疗良性前列腺增生 (BPH)的远期疗效。方法 :BPH患者 80例 ,根据治疗方法分为TUVP组和经尿道前列腺电切术 (TURP)组 ,比较其国际前列腺症状评分 (IPSS)、生活质量评分 (QOL) ,最大尿流率Qmax、膀胱剩余尿量 (PRV)及远期并发症 (尿道狭窄、尿失禁及再次手术 )的发生率等情况。结果 :TUVP与TURP患者的各项指标术前分别为IPSS(2 9.7± 4 .3)和 (2 8.6± 4 .5 )分、QOL(5 .7± 0 .8)和 (5 .3± 0 .7)分、Qmax(8.4± 3.9)和 (8.2± 4 .2 )ml/s、PRV(2 4 3.0± 5 5 .3)和 (2 4 0 .0± 4 7.2 )ml,均P值 >0 .0 5。术后分别为IPSS(8.1± 1.7)和 (10 .7± 2 .6 )分 ,QOL(1.7± 0 .6 )和 (2 .0± 0 .5 )分 ,Qmax(2 1.6± 4 .3)和 (19.1±4 .3)ml/s、PRV(2 8.7± 10 .4 )和 (38.7± 12 .5 )ml,均P值 >0 .0 5。远期并发症的发生率分别为 5 .2 % (TUVP)和7.1% (TURP) ,P值 >0 .0 5。结论

Purpose:To compare the safety, efficacy and durability of transurethral electrovaporization of the prostate (TUVP) with transurethral resection of the prostate (TURP) as gold standard. Methods:80 patients admitted from surgery for BPH were randomized to either TUVP group 〔38 patients mean age ( 69.3 ± 4.8 )〕 or TURP group 〔42 patients, mean age ( 67.8 ± 5.3 ) 〕after 2 years of follow up, respectively. Using the international prostate symptom score (IPSS), the associated quality of life score (QOL), postvoid residual volume (PRV) and maximum urinary flow rate (Q max ). Results:Both groups had comparable mean IPSS, QOL Q max and PRV respectively, preoperation in IPSS at ( 29.7 ± 4.3 ) vs ( 28.6 ± 4.5 ), in QOL at ( 5.7 ± 0.8 ) vs ( 5.3 ± 0.7 ), in Q max at ( 8.4 ± 3.9 )ml/s vs ( 8.2 ± 4.2 )ml/s, in PRV at ( 243.0 ± 55.3 )ml vs ( 240.0 ± 47.2 )ml (P> 0.05 ); postoperation in IPSS at ( 8.1 ± 1.7 ) vs ( 10.7 ± 2.6 ), in QOL at ( 1.7 ± 0.6 ) vs ( 2.0 ± 0.5 ), in Q max at ( 21.6 ± 4.3 )ml/s vs ( 19.1 ± 4.3 )ml/s, in PRV at ( 28.7 ± 10.4 )ml vs ( 38.7 ± 12.5 )ml (P> 0.05 ). Incidence rate of long term complications in 5.2 % of the TUVP group and 7.1 % of the TURP group (P> 0.05 ). Conclusions:The 2 year follow up outcome confirms that TUVP is as effective as standard TURP in the treatment of moderate sized BPH.
