双套管持续负压冲洗法对直肠癌低位前切除术后吻合口瘘的防治研究被引量：6

摘要目的探讨双套管持续负压冲洗法防治直肠癌低位前切除术后吻合口瘘的临床疗效。方法回顾性分析217例数据完整的开腹直肠癌低位前切除术患者的临床资料,其中110例采用简易双套管持续负压冲洗法防治术后吻合口瘘(观察组),107例采用传统腹腔引流方法防治术后吻合口瘘(对照组)。对比2组患者吻合口瘘发生率以及手术干预率、引流管放置时间、住院时间、吻合口狭窄发生率。结果全部217例患者术后吻合口瘘发生率为4.0%(8/217),其中观察组4例(3.63%,4/110),对照组4例(3.7%,4/107),差异无统计学意义(P>0.05)。观察组和对照组患者吻合口瘘的发生时间均为(7.3±1.4)d,差异无统计学意义(P>0.05)。观察组患者均未再次行手术干预,而对照组有5例经保守治疗无效而行手术治疗,差异有统计学意义(P<0.05)。观察组和对照组引流管放置时间分别为(9.7±2.7)d和(16.4±3.6)d,比较差异有统计学意义(P<0.05)。住院时间分别为(15.7±4.3)d和(21.5±6.4)d(P<0.05),比较差异有统计学意义。结论自制简易双套管不能降低直肠癌低位前切除术后吻合口瘘发生率,但可提高吻合口瘘保守治疗成功率,降低吻合口瘘手术干预率,提高患者生活质量。
