探讨血流向量成像技术对房间隔缺损患者右心房和右心室内流场动态变化的临床意义 被引量:3

Investigate the Clinical Significance of Flow Field Dynamic Changes of Right Atrium and Right Ventricle in Patients with Atrial Septal Defect by Vector Flow Mapping

摘要 目的探讨血流向量成像技术(vector flow mapping,VFM)对房间隔缺损(atrial septal defect,ASD)患者右心房和右心室血流流场成像,与正常人对比分析心腔内心动周期不同时相的血流状态的变化特点及临床意义。方法选取50例ASD患者为研究组,53例健康志愿者为对照组,动态分析心腔内涡流图、向量图及流线图的变化规律,测量涡流直径(横径Dx、纵径Dy)及边缘最大速度(maximum speed,Vmax)将对照组与研究组进行比较,采用t检验。结果研究组在涡流大小及边缘速度与对照组比较,在右心房、右心室内显著增高,具有统计学意义(P<0.05),而向量图及流线图均有明显增加。结论 VFM图像能显示ASD患者右心房、右心室内血流流场变化,为深入研究ASD患者血流动力学特点和异常变化提供初步理论依据。 Objective To detect blood flow field imaging of right heart in pPatients with atrial septal defect(ASD)and in healthy volunteers by vector flow mapping(VFM)and to analyze the clinic significance of cardiac chamber flow field change at different phases of the cardiac cycle.Methods Fifty patients with ASD were involved in the experimental group and 53 healthy volunteers were involved in the control group.The technology of VFM was used to measure vortex diameter(transverse diameter,Dx,longitudinal diameter Dy)and maximum speed(Vmax)of the edge in the control group and the experimental group and to analyze changes of vortex maps,vector graphics and streamline map.Result The vortex size and edge vortex velocity of right atrium and right ventricle were significantly increased in the experimental group(P<0.05)compared with the control group as well as the vector diagrams and flow charts.Conclusion VFM technology can demonstrate the blood flow field changes of right heart in patients with ASD,which provides a preliminary theoretical foundation for a further study of intracardiac hemodynamics characteristics in ASD patients.
