摘要
目的通过对正常成人完整骨盆电子计算机断层扫描(computed tomography,CT)三维重建数据进行数字化测量,获得正常成人髂嵴水平面偏转角度,为髂骨骨折临床复位提供理论支持。方法收集2016-06/2018-06月于作者医院行CT检查的60例(120侧)正常成人完整骨盆CT断层扫描数据,应用Mimics 20.0和Geomagic Studio 12.0软件对骨盆数据进行三维重建,对髂嵴水平面偏转角度进行测量。结果髂嵴水平面呈类"S"型,左右侧偏转角度对比差异无统计学意义(P>0.05)。男性和女性前中部髂嵴偏转角度分别为(134.58±8.96)°和(131.77±8.39)°,中后部髂嵴偏转角度分别为(150.77±9.03)°和(148.19±8.64)°,差异均无统计学意义(P>0.05)。中后部偏转角度β明显大于前中部偏转角度α,差异有统计学意义(P<0.001)。结论正常成人髂嵴水平面呈类"S"型,中后部偏转角度明显大于前中部偏转角度,弥补了髂骨解剖参数的不足,有利于髂骨骨折的复位内固定。
Objective To obtain iliac crest horizontal deflection angle of normal adult by three-dimensional(3 D)computed tomography(CT)reconstruction and digital measurement of adult intact pelvis,and provide theoretical support for clinical reduction of iliac fracture.Methods Pelvic CT scan data of 60 adults(120 sides)in authors′hospital from June 2016 to June 2018 were collected.The 3 D pelvic model was reconstructed and the iliac crest horizontal deflection angle was measured using Mimics 20.0 and Geomagic Studio 12.0 software.Results The horizontal plane of the iliac crest was S-like,and there was no significant difference in the left-right deflection angle of pelvis(P>0.05).The anterior-middle iliac crest deflection angles of males and females were(134.58±8.96)°and(131.77±8.39)°,respectively,and the middle-posterior iliac crest deflection angles were(150.77±9.03)°and(148.19±8.64)°,respectively.And there were no significant differences between males and females(P>0.05).The middle-posterior deflection angleβwas significantly greater than the anterior-middle deflection angleα(P<0.001).Conclusion The iliac crest horizontal plane of normal adults is S-like,and the middle-posterior deflection angle is significantly greater than the anterior-middle deflection angle,which makes up for the lack of iliac bone anatomical parameters and is conducive to the reduction and internal fixation of iliac bone fracture.
