全氟显在子宫输卵管四维超声造影中评价输卵管通畅性的临床应用 被引量:5

Clinical Application of Four-dimensional Hysterosalpingo-contrast Sonography with Fluorocarbon in Evaluating Fallopian Tube Patency

摘要 目的探讨新型造影剂全氟显低机械指数实时谐波经阴道四维超声子宫输卵管造影(four-dimensional hysterosalpingo-contrast sonography,4D-HyCoSy)诊断输卵管通畅性的临床应用价值。方法回顾性分析2014-01/2017-06月在作者医院行4D-HyCoSy检查的124例不孕患者的临床资料,将腹腔镜检查结果作为金标准,评估4D-HyCoSy诊断输卵管通畅性的准确性。结果 4D-HyCoSy诊断输卵管通畅的特异度97.69%(127/130)、敏感度91.96%(103/112);通而不畅的特异度94.32%(166/176)、敏感度90.91%(60/66);阻塞的特异度96.07%(171/178)、敏感度92.19%(59/64);4D-HyCoS与腹腔镜诊断的一致性较好(Kappa值为0. 872),结果差异无统计学意义(P>0.05)。结论国产造影剂全氟显4D-HyCoSy诊断输卵管图像清晰、直观,判断准确,方便、安全、费用低,可作为临床不孕症患者输卵管通畅性检查的有效方法。 Objective To explore the clinical value of transvaginal four-dimensional hysterosalpingo-contrast sonography(4 D-Hycosy)with new contrast agent fluorocarbon in diagnosis of the fallopian tube patency.Methods A total of 124 infertile patients underwent 4 D-Hycosy examination in authors’hospital from January 2014 to June 2017 were retrospectively analyzed.The examination results of laparoscopy were used as gold standard.The veracity of 4 D-Hycosy in diagnosis of the fallopian tube patency was evaluated.Results The diagnostic specificity and sensitivity of oviducts unobstructed with 4 D-HyCoSy was 97.69%(127/130)and 91.96%(103/112),partially unobstructed was 94.32%(166/176)and 90.91%(60/66),obstructed was 96.07%(171/178)and 92.19%(59/64).The consistency of 4 D-HyCoSy and laparoscopy in diagnosis of fallopian tube patency was good(Kappa=0.887).Conclusion 4 D-Hycosy with new contrast agent fluorocarbon can be an effective method in the evaluation of the fallopian tube patency for infertil women,which is accurate,convenient,safe and low cost,and it is worth for clinical promotion.
