幕上脑动静脉畸形破裂出血显微手术51例分析

Experience and skill of microsurgery on treating patients with supratentorial cerebral arteriovenous malformation

目的探讨脑动静脉畸形(AVM)显微手术治疗的经验和技巧。方法采用回顾性分析方法对51例幕上脑动静脉畸形破裂出血患者的临床资料进行收集、整理和分析。结果所有病人均进行血肿清除及畸形血管团显微切除手术,其中46例为全切除,5例为次全切除后行γ刀放射治疗。术后病人恢复良好。结论充分的术前影像学分析、个体化手术方案设计及术中监控非常重要,熟练的显微手术操作技巧是手术成功的关键。

Objective To investigate the experience and skill of microsurgery on treating patients with supratentorial cere-bral arteriovenous malformation (SCAVM )· Methods The data of 51 patients with SCAVM were retrospectively ana-lyzed·Results 46 cases of patients were treated with hematoma and arteriovenous malformation microsurgical complete resec-tion ,and 5 cases of patients were treated with hematoma and arteriovenous malformation microsurgical incomplete resection plusγknife radiotherapy· All patients recovered well· Conclusion Adequate preoperative imaging analysis ,individualized sur-gical plan design and intra-operative monitoring is very important in succeed operation ,especially in adept microsurgical opera-tive skills.
