消化性溃疡胃镜检查结果临床分析

The Endoscopic and Clinical Analysis of Peptic Ulcer Diseases

目的:了解长沙地区近三年来消化性溃疡的发病特点及与几个相关因素的关系。方法:对湖南省人民医院2010年3月~2013年3月三年来经胃镜检查诊断为消化性溃疡(PU)的病例资料进行回顾性分析。结果:(1)三年共行胃镜检查23593例,诊断为PU的为3765例,检出率为15.96%,其中DU为2604例,GU为834例,其他溃疡327例;(2)PU患者平均年龄为47.6岁,DU以41~60岁居多;GU及其他溃疡以50岁以上居多;(3)PU患者男女之比为2.29:1,其中DU男女之比为2.11:1,GU男女之比为2.52:1;(4)DU与GU两者之比为3.12:1;(5)溃疡大小:直径小于1cm为2641例,1~2cm为969例,大于2cm为155例,(6)PU患者幽门螺旋杆菌(HP)的检出率为72.56%。结论:(1)消化性溃疡以十二指肠溃疡最多,胃溃疡次之,其他溃疡最少;(2)十二指肠溃疡好发于中年,胃溃疡和其他溃疡以中老年居多;(3)消化性溃疡发病率男性明显高于女性;(4)良性溃疡大小多在2cm以下;(5)PU幽门螺旋杆菌检出率较以往减少,但仍是引起消化性溃疡的主要原因之一。

Objective To understand in recent years, Changsha region of peptic ulcer disease and several re-lated factors. Methods Retrospective analysis of cases diagnosed by endoscopy in our hospital from March 2010 to March 2013 for three years for PU. Results (1) three years A total of gastroscopy 23,593 cases diagnosed as PU 3765 cases, the detection rate of 15.96% DU for 2604 cases, 834 cases in the GU, 327 cases of other ulcer, (2) PU average patient age was 47.6 years, of DU mostly 41 to 50 years old;the GU and other ulcers are mostly above 50 years of age, (3) ulcers male and female ratio of 2.29:1, the DU male to female ratio of 2.11:1, the GU for men and women ratio of 2.52:1, (4) DU and GU between the ratio of about 3.12:1, (5) ulcer diameter less than 1cm of 2641 cases, 1-2cm for the 969 cases, more than 2cm of 155 cases, ( 6) in patients with Helicobacter pylori (HP)-positive rate was 72.56%. Conclusion (1) peptic ulcer, duodenal ulcer up to, followed by gastric ulcer, other ulcer at least (2) duodenal ulcer occurs in young adults, ulcers and other ulcers, mostly middle-aged; ( 3) peptic ulcer incidence of male than female, (4) benign ulcer size more than 2cm below, (5) Helicobacter pylori is still one of the main cause of peptic ulcer.
