胆囊癌患者手术前后血清HIF-1α水平的变化特点及临床意义

Changes of serum HIF-1 levels before and after operation in patients with gallbladder carcinoma and its clinical significance

目的 :了解胆囊癌患者胆囊癌根治手术及复发转移后HIF-1α的变化特点和临床意义。方法 :选取我院2010年1月~2011年9月收治的胆囊癌患者,其中男36例,女52例,用酶联免疫吸附试验法检测患者手术前1天、术后1周和术后2周的血清HIF-1α水平,观察其动态变化,分别记录其复发转移情况,比较胆囊癌复发转移患者治疗后2周与复发转移时血清HIF-1α含量,通过血清HIF-1α含量判断胆囊癌患者手术治疗效果及预测预后的价值进行评价。结果 :术后1周较术前1天明显降低,且术后2周较术后1周稍有下降。血清HIF-1α水平增高与胆囊癌患者TNM分期Ⅰ~Ⅳ分级、肿瘤体积及肿瘤转移的阴阳性率密切相关,但两组的HIF-1α值在年龄(50岁为界限)及性别方面无明显差异。结论 :HIF-1α表达与胆囊癌病人疾病进展明显相关,检测患者血清HIF-1α的动态变化,可以作为评判患者病情的有效临床生化指标,灵敏度和准确度均较高,可为制定个体化手术方案提供依据,具有重要的临床应用价值。

Objective To explore the Changes and clinical significance of serum HIF-1 levels before and after operation in patients with gallbladder carcinoma.Methods In our hospital from 2010 January to 2011 September were in patients with gallbladder carcinoma,including 36 cases of male,female 52 cases,using enzyme linked immunosorbent assay in detection of patients before operation,1 days,1 weeks after operation,the serum HIF-1 level in 2 week after operation,observe the dynamic changes of the recurrence,were recorded.Shift,compared to treatment of patients with metastatic carcinoma of gallbladder recurrence after 2 weeks and HIF-1 serum content of recurrence and metastasis,serum HIF-1 content by judging therapeutic effect in patients with carcinoma of gallbladder operation and prognostic evaluation.Results 1 weeks after operation than that before operation 1 days significantly reduced,the difference was significant,and 2 weeks after the operation,a slight decline in 1 weeks after operation,there was no significant difference.Serum HIF-1 levels were increased in patients with gallbladder cancer TNM stage I-IV grade,tumor size and tumor metastasis is closely related to the positive and negative rate,the difference was statistically significant,but the two groups of HIF-1 values in the age(50 years old as the boundary) and gender had no significant difference.Conclusion HIF-1 expression was significantly related to development and disease in patients with gallbladder carcinoma,detection of dynamic changes of serum HIF-1 in patients with alpha,effective clinical biochemical indexes can be used as evaluation of the patient’s condition,high sensitivity and accuracy,which can provide the basis for the development of individualized surgical plan,it has important value in clinical application.
