双侧钢板内固定治疗复杂胫骨平台骨折的临床效果及对患者术后功能的影响

Clinical effect of bilateral plate fixation for the treatment of complex fractures of the tibial plateau and the effect on the function of patients after operation

目的 :探索双侧钢板内固定治疗复杂胫骨平台骨折的临床疗效及对患者术后功能的影响。方法 :将确诊为复杂胫骨平台骨折的60名患者,随机分成对照组30例患者采用单侧钢板内固定治疗,治疗组30例患者采用双侧钢板内固定治疗,观察对比两组患者手术用时、手术中出血量、住院时间、手术后的并发症、骨折愈合时间、临床疗效及术后功能变化情况。结果 :治疗组患者手术用时、手术出血量和住院时间明显长于对照患者,骨折愈合时间明显短于对照组,两组差异具有统计学意义;治疗组患者术后并发症发生率为10.0%,对照组患者术后并发症发生率为36.7%,两组差异具有统计学意义;对照组患者复位优良率为46.7%,功能恢复优良率为43.3%,治疗组患者复位优良率为90.0%,功能恢复优良率为93.3%,两组样本数据差异具有统计学意义。结论 :采用双侧钢板内固定治疗复杂胫骨平台骨折患者的临床疗效显著,在骨折复位情况、功能恢复情况方面均优于采用单侧钢板内固定治疗的效果,并且治疗组患者治疗后的术后功能明显优于对照组患者,在临床治疗复杂胫骨平台骨折患者值得进一步推广应用。

Objective To explore the clinical effect of bilateral plate fixation for the treatment of complex fractures of the tibial plateau and the effect on the function of patients after operation.Methods Diagnosed 60 patients complicated tibial plateau fracture,were randomly divided into control group of 30 patients with unilateral plate internal fixation for the treatment,treatment group 30 cases were treated by bilateral plate fixation for the treatment of bleeding volume,operation time,hospitalization time,hand,postoperative complications,fracture healing and functional changes of clinical curative effect of the operation time after observation,compared two groups of patients with operation.Results The patients treated with surgery,the surgical blood loss and length of hospital stay was significantly longer than control patients,fracture healing time was significantly shorter than the control group,the difference was statistically significant(P<0.05); The incidence of postoperative complications in patients treated 10.0%,postoperative complications occurred in the control group was 36.7%,the difference was statistically significant(P<0.05); Patients in the control group reset the excellent and good rate was 46.7%,the excellent rate of functional recovery of the patients in the treatment group was 43.3%,reset the excellent and good rate was 90%,the rate of excellent and good functional recovery was 93.3%,with statistically significant difference between the two groups of sample data(P<0.05).Conclusion With bilateral plate fixation for the treatment of complex tibial plateau fractures in patients with clinical efficacy significantly,in the reduction of fracture,recovery of function are better than those with unilateral plate internal fixation treatment effect,patients in the control group and the treatment group patients after the treatment of postoperative function was significantly better,in the clinical treatment of complex tibial plateau fracture patients deserves further popularization and application.
