人工流产妇女意外妊娠现状分析及临床干预的研究

The Study of the Present Situation and Clinical Intervention of Unexpected Pregnancy after Artificial Abortion in 304 patients

目的 :探讨人工流产妇女意外妊娠的临床特征及临床干预效果。方法 :产科门诊手术室进行临床干预前161例(对照组)与临床干预后143例(观察组)接受人工流产患者作为研究对象,自2013年3月起实施临床干预措施,对比两组患者术后再次意外妊娠发生率、避孕知识及生殖健康知识掌握情况、术后不良反应及不良心理情绪发生率。同时根据术后是否再次妊娠分为两组,对比两组患者年龄、户口性质、经济收入、文化程度、是否未婚同居、性伴侣数量、首次性行为年龄等。结果 :年龄小于19岁、农村户口、月收入在2000元以下、产妇高中以下文化程度、未婚同居、性伴侣在2个以上和首次性行为在17岁以下人工流产术后患者再次意外妊娠发生率显著增高;观察组避孕知识及生殖健康知识知晓率均显著高于对照组;术后再次意外妊娠率、术后不良反应发生率以及术后不良心理发生率均显著低于对照组。结论 :掌握人流术后再次意外妊娠的高危因素以及正确的临床干预对于降低术后再次意外妊娠率、有效降低患者术后不良反应及不良情绪的发生率均有重要意义。

Objective To discuss the present situation and clinical intervention of unexpected pregnancy after artificial abortion in 304 patients. Methods 161 patients before(the control group) and 143 patients after(the observation group) the implementation of clinical intervention undergone artificial abortion were analyzed, the unexpected pregnancy rate after artificial abortion, contraceptive knowledge and reproductive healthy knowledge, postoperative adverse reactions and adverse psychological occurrence rate were contrasted between the two groups. Then 304 patients were divided into two groups according to with or without unexpected pregnancy after artificial abortion to discuss the related factors. Results The pregnancy rate after the accidental in patients less than 19 years, rural registered permanent residence, monthly income less than 2000 yuan, less than high school education, the unmarried cohabitation, sexual partners more than 2 and the first sexual behavior at the age of 17 years were increased significantly; master rate of contraceptive knowledge and awareness rate of reproductive healthy knowledge in the observation group were significantly higher than the control group; unexpected pregnancy rate 1 year after artificial abortion, postoperative adverse reactions and adverse psychological occurrence rate in the observation group were significantly lower than the control group. Conclusion Master the risk factors of unexpected pregnancy after artificial abortion and correctly clinical intervention can reduce postoperative unexpected pregnancy rate, effectively reduce the postoperative adverse reactions and adverse emotional occurrence rate has important significance.
