摘要 目的 :分别检测宫颈分泌物、血液和宫颈病变组织中的基质金属蛋白酶-9(MMP-9)和转化生长因子β1(TGF-βl)的表达情况,探讨其在宫颈病变形成及演进过程中的意义。方法 :应用ELISA法检测66例病人(其中慢性宫颈炎组织10例、CINcervical intraepithelial neoplasia宫颈上皮内瘤变24例,宫颈鳞癌32例)的宫颈分泌物和血清中的TGF-βl和MMP-9的表达水平。应用免疫组织化学SP法检测相对应病人石蜡包埋的不同病变宫颈组织中TGF-βl和MMP-9的表达。结果 :1.在血清和宫颈分泌物中TGF-βl和MMP-9的浓度与宫颈癌密切相关:慢性宫颈炎组、宫颈上皮内瘤变(CIN)组和宫颈鳞癌(Cx Ca)组的样本中TGF-βl和MMP-9的浓度逐级升高,提示随着宫颈癌恶性程度的增加,TGF-βl和MMP-9的表达明显增强。血清和宫颈分泌物中MMP-9和TGF-β1与其切片免疫组化结果相吻合。随着MMP-9在宫颈分泌物和血清中表达的增强,TGF-β1的表达也明显增高,两者呈正相关。2.TGF-βl和MMP-9表达与宫颈癌的病理和临床分类的关系:在宫颈癌组织中有淋巴结转移的MMP-9和TGFβ1的表达阳性率显著高于无淋巴结转移的。在宫颈癌≥Ⅱb期的MMP-9和TGF-βl的表达阳性率显著高于≤Ⅱa期的。MMP-9和TGF-βl与宫颈癌肿瘤的大小和组织病理学分级无显著性差异。结论 :TGF-βl和MMP-9在宫颈病变、宫颈分泌物和血清的中表达与病变良恶性有关,TGF-βl和MMP-9高表达是宫颈恶性肿瘤的表现之一,并且TGF-βl和MMP-9在宫颈分泌物或血清中的表达具有显著相关性。 Objesctive The expression of matrix metalloproteinase-9 (MMP-9) and transforming growth factorβ1 (TGF-βl) were detectedin cervical secretions, blood andcervical lesions, in order to investigate the significance in thedevelopment ofcervical lesions. Methods 66 cases (including 10 cases of chronic cervicitis tissues, 24 cases of cervical intraepithelial neo-plasia cervical intraepithelial neoplasia (CIN) and 32 cases of cervical squamous cell carcinoma) were detected by ELISA, to investigate the expression of TGF-β1 and MMP-9 in cervical secretions and serum. The expression of TGF-βl and MMP-9 in paraffin-embedded of cervical lesions samples were detected by SP method of immunohistochemical assay. Results 1. The concentration of TGF-βlandMMP-9 in serum andcervicalsecretionsare closely associatedwithcervical cancer: the concentra-tion of TGF-βlandMMP-9 in chronic cervicitis group, cervical intraepithelial neoplasia(CIN) groupand cervicalsquamous cell carcinomagroup(CxCa)groupprogressivelyincreased, suggestingincreasedwith thedegree of malignancyof cervical cancer, the expression of TGF-βl andMMP-9were significantlyincreased. The MMP-9andTGF-β1 in serum andcervical secretions were consistent with the results in immunohistochemistryof their microtome. With the expression of MMP-9 enhanced in cervicalse-cretionsand serum,the expression of TGF-β1alsoincreased significantly, the correlation of them were positive. 2. The relation-ship between the expression in TGF-βlandMMP-9andthe pathologicaland clinicalclassification ofcervical cancer:the expres-sion of MMP-9and TGF-β1 in cervicallymph node metastasis tissue were significantlyhigher than those withoutlymph node metastasis. The expression of MMP-9and TGF-β1 in ≥Ⅱb periodin cervical cancerwere significantlyhigher than that in ≤Ⅱa period.There were no significantdifference between expression of TGF-β1,MMP-9andcervicaltumor size andhistopathologic grade of cervical cancer. Conclusion The expression of TGF-βlandMMP-9in cervical lesions, cervical secretionsand serum are related with the degree of benign and malignantlesions.The higher expression of TGF-β1 and MMP-9are one of the mani-festationsof cervicalcancer.
