回收式自体输血在各类手术中的应用情况分析

Application analysis of intraoperative blood salvage in various kinds of surgical operations in Changsha Central Hospital

目的 :分析回收式自体输血在各类手术中的使用频率,发现回收式自体输血使用的优势术种,了解该技术在所有手术中使用的概况。方法：回顾性收集我院2012年1月~2015年5月全部术中用血病例(包括回收式自体输血和异体输血),按不同手术分类(27类)。对自体血液回输和异体血液使用例次进行统计,分析回收式自体输血在各类手术中的情况。结果 :回收式自体输血(522例)占同期术中(1211例)用血例次的43.10%,回输血量363170m l,占术中用血量的35.21%。其中,分布于神经外科手术(包括颅骨修补,颅脑外伤和非创伤性颅内出血)236例,异位妊娠手术69例,非感染类脊柱手术39例,骨折内固定手术42例,髋关节置换术24例,胸、腹部闭合性损伤手术91例;以上8类手术中应用自体血液回输例次占术中用血总例次的61.17%(501/819),占同期自体血回输例次(522)的95.98%。结论 :回收式自体输血在神经外科等8类手术中有较高的使用频率,特别在急诊手术中具有其优势。充分理解和遵循自体血液回输适应证将进一步提高其临床应用价值。

Objective By analysis of the application frequency of intraoperative blood salvage (IBS) in various kinds of surgical operations, to discover the most benefited operation kinds, and to understand the situation of IBS in all kinds of surgi-cal operations. Methods All retrospective cases (including salvage autologous and allogeneic blood transfusion) from January, 2012 to May, 2015 in Changsha Central Hospital were collected and classified according to surgical operation classification (27 kinds). Cases of autologous and allogeneic blood transfusion were counted to analyze the situation of IBS in all kinds of surgical operations. Results 522 cases of IBS accounted for 43.10% of all blood transfusion in the same period. The amount of IBS blood transfusion was 363170ml, which accounted for 35.21% of all blood transfusions. Among them, there were 236 cases of neuro-surgical operations, including skull repairing, craniocerebral trauma, and non traumatic intracranial hemorrhage cases, 69 cases of ectopic pregnancy operations, 39 cases of non-infectious spinal operation, 42 cases of fracture internal fixation operation, 24 cases of total hip arthroplasty operation, and 91 cases of thoracic and abdominal closed injury operations. The IBS cases from <br> these 8 kinds of operations accounted for 61.17% (501/819) of all blood transfusions, and accounted for 95.98% of all IBS blood transfusions (522 cases). Conclusion The IBS has higher application frequency in the 8 kinds of operations including neuro-surgical operations. It has especial advantages in emergency operations. Fully understand and follow the indication of IBS will further improve its clinical application value.
