早期切开复位无头加压空心螺钉及克氏针治疗经舟骨月骨周围脱位的临床研究

Early open reduction of headless compression screws and Kirschner wire for the treatment of the clinical study of scaphoid perilunate dislocation

目的:探讨经舟骨月骨周围脱位患者实施早期切开复位加克氏针联合加压空心螺钉内固定治疗的临床效果。方法:从我院2012年7月2014年9月收治的经舟骨月骨周围脱位患者72例作为研究对象,采用随机数字表法分为观察组与对照组,每组各36例,均实施早期切开复位,对照组在此基础上行单纯克氏针内固定治疗,观察组联合加压空心螺钉固定,观察两组患者治疗效果、术后恢复情况以及并发症发生情况。结果:观察组患者治疗优良率为91.67%,与对照组患者77.78%相比较,显著较高;观察组患者术后肿胀消除时间、骨折愈合时间同对照组比较,均明显较短,腕关节活动范围明显较对照组大;观察组并发症发生率为11.11%,同对照组22.22%比较,明显较低。结论:对于经舟骨月骨周围脱位患者,实施早期切开复位加克氏针联合加压空心螺钉内固定治疗,疗效确切,可有效缩短患者康复时间,促进骨折愈合,减少并发症发生,值得推广。

Objective To explore by scaphoid perilunate dislocation were treated with implementation early open reduction and Kirschner wire and compression hollow screw internal fixation for the treatment of clinical effect. Methods from our hospital in July, 2012 2014 September were 72 cases of scaphoid perilunate dislocation were treated by as the research object, the ran-dom number table method is divided into observation group and control group, each with 36 cases in each, the implementation of the early open reduction, control group on the basis of upward only gram needle fixation for the treatment of observed group and compression hollow screw fixation, patients in the two groups were observed after treatment of clinical effect and recovery and complications incidence. Results the excellent and good rate of the observation group was 77.78%, compared with the con-trol group. The patients with the observation group were significantly lower than those in the control group, and the incidence of complications was 11.11%, the observation group was significantly lower. Conclusion for the scaphoid perilunate dislocation were treated, the implementation of early open reduction and Kirschner wire and compression hollow screw internal fixation for the treatment, curative effect is exact, which can effectively shorten the recovery time of patients, and promote fracture healing, reduce complications, worthy of promotion.
