三线减张鼻中隔矫正联合低温等离子RFA治疗伴有鼻中隔偏曲的中-重度PAR患者的效果

The effect of three line tension nasal septum correction combination with plasma RFA in moderate-severe PAR patients with nasal septum deviation

目的 :探讨三线减张鼻中隔矫正联合低温等离子射频消融术(RFA)治疗伴有鼻中隔偏曲的中-重度变应性鼻炎(PAR)患者的效果。方法 :收集在我院行手术治疗的鼻中隔偏曲的中重度持续性变应性鼻炎患者87例,根据三线减张鼻中隔矫正是否联合低温等离子射频消融术治疗,分为观察组和对照组,比较两组手术前后VAS评分、治疗效果和嗜酸性粒细胞计数的差异。分析治疗前后VAS评分与嗜酸性粒细胞计数的相关关系。结果 :两组手术后6个月和手术后12个月的VAS评分均低于手术前,且差异具有统计学意义;观察组手术后6个月和手术后12个月的VAS评分(1.55±0.41,0.74±0.14)低于对照组(2.28±0.32,1.33±0.21),且差异具有统计学意义。观察组手术后治疗有效率(44例,93.62%)高于对照组(28例,70.0%),且差异具有统计学意义。两组手术后6个月和手术后12个月的嗜酸性粒细胞计数均低于手术前,且差异具有统计学意义;观察组手术后6个月的嗜酸性粒细胞计数(0.38±0.14)低于对照组(0.57±0.22),且差异具有统计学意义。患者手术前、术后6个月、术后12个月的VAS评分与嗜酸性粒细胞计数均呈正相关。结论 :三线减张鼻中隔矫正联合低温等离子射频消融术治疗鼻中隔偏曲的中-重度变应性鼻炎效果优于三线减张鼻中隔矫正术。

Objective To investigate the effect of three line tension nasal septum correction combination with plasma RFA in moderate-severe PAR patients with nasal septum deviation. Methods Collected 87 cases of moderate-severe persistent allergic rhinitis patients with nasal septum deviation in our hospital, according to the three line tension nasal septum whether or not combined with plasma RFA was divided into observation group and control group, VAS score, treatment effect and eosinophil count between two groups before and after surgery were compared, the correlated analysis of VAS score and eosinophil count before and after treatment. Results VAS scores of postoperative 6 months and 12 months in the two groups were significantly lower than that before operation, and the difference was statistically significant; The VAS score of postoperative 6 months and 12 months in the observation(1.55 + 0.41, 0.74 + 0.14)was lower than that of control group(2.28 + 0.32, 1.33 + 0.21), and the difference was statistically significant. The treatment efficiency of observation group after the operation(44 cases, 93.62%)was higher than the control group(28 cases, 70%), and the difference was statistically significant. The eosinophil count of postoperative 6 months and 12 months in the two groups were significantly lower than that before operation, and the difference was statistically significant; The eosinophil count of postoperative 6 months in the observation group(0.38 + 0.14)was lower than that of control group(0.57 + 0.22), and the difference was statistically significant. The VAS score of preoperative and postoperative 6 month, 12 months after surgery, and eosinophil count were positively correlated. Conclusion The effect of three line tension nasal septum correction combination with plasma RFA treatment of moderate-severe PAR patients with nasal septum deviation was better than that of single three line tension nasal septum correction.
