关节镜辅助下带袢钢板(tight-rope)内固定治疗肩锁关节脱位的应用研究

目的:研究关节镜辅助下带袢钢板(tight-rope)内固定治疗肩锁关节脱位中的应用效果。方法:选取2015年3月~2017年1月期间在我院接受诊治的100例肩锁关节脱位患者作为研究对象,根据随机数表法分为对照组、观察组,各50例。对照组接受锁骨钩钢板固定治疗,观察组实施关节镜辅助下带袢钢板内固定治疗。统计比较两组患者的手术时间、住院时间、出血量、疼痛程度以及肩关节内旋、外旋、外展活动度。结果:观察组手术时间、术中出血量、术后住院时间均低于对照组,差异有统计学意义;术后两组患者疼痛评分均低于术前,且观察组低于对照组,差异有统计学意义;术后3个月观察组患者肩关节内旋、外旋、外展活动度均高于对照组,差异有统计学意义。结论:关节镜辅助下带袢钢板内固定治疗肩锁关节脱位损伤小,能有效减轻肩关节疼痛,改善肩关节活动度,利于功能恢复。

Objective To study the effect of arthroscopic assisted tightening with tight-rope internal fixation for acromioclavicular joint dislocation. Methods 100 cases of patients with acromioclavicular dislocation who were treated in our hospital from March 2015 to October 2016 were selected as subjects. According to the random number table method was divided into control group and observation group, each group of 50 cases. The control group received clavicular hook plate fixation, and the observation group was treated with arthroscopic assisted plate internal fixation. The duration of surgery, hospitalization, bleeding, and pain were observed in both groups As well as the rotation of the shoulder joint, external rotation, abduction activities were compared between the two groups. Results The operative time, intraoperative blood loss and postoperative hospital stay were lower in the observation group than in the control group. The pain scores of the two groups were lower than those before operation, and the observation group was lower than the control group, the difference was statistically significant; The internal rotation, external rotation and abduction activity of the shoulder were significantly higher than those of the control group at 3 months after operation. Conclusion Arthroscopic assisted plate internal fixation treatment of acromioclavicular joint dislocation injury is small, can effectively reduce shoulder pain, improve shoulder mobility, conducive to functional recovery.
