婴儿肝炎综合征患儿CMV和TOX感染的检测分析

Analysis of CMV and TOX infection for infantile hepatitis syndrome

摘要 目的:探讨婴儿肝炎综合征(IHS)患儿巨细胞病毒(CMV)和弓形虫(TOX)感染情况。方法:采用ELISA检测76例IHS患儿及对照组30例婴儿血清CMV-IgM、TOX-IgM、IL-6和TNF-a的水平,采用全自动生化分析仪检测血清中的TBIL、DBIL、TBA、ALT和γ-GT水平。结果:76例IHS患儿血清CMV-IgM阳性率59.21%(45/76),30例对照婴儿CMV-IgM阳性率为3.33%(1/30)。76例IHS患儿血清TOX-IgM阳性率9.21%(7/76),30例对照婴儿TOX-IgM检测结果均阴性,统计学分析显示IHS患儿血清CMV-IgM、TOX-IgM阳性率均明显高于对照组,差异有统计学意义。76例IHS患儿血清TBIL、DBIL、TBA、ALT、γ-GT、IL-6和TNF-a水平均明明显高于对照组。结论:CMV感染是引起IHS的首要原因,TOX等寄生虫感染是部分HIS的重要致病因子。血清中TBIL、DBIL、TBA、ALT、γ-GT、IL-6和TNF-a水平的明显增高与IHS的病损程度密切相关,检测上述血清因子可为HIS病情判断和预后评估提供依据。 Objective To explore the relationship between infantile hepatitis syndrome(IHS) and cytomegalovirus(CMV) and toxoplasma(TOX), the changes of serum constituents. Methods The constituents from 76 IHS subjects and 30 controls, which CMV-IgM、TOX-IgM、IL-6 and TNF in serum were detected by enzyme-linked immunosorbent assay(ELISA), including TBIL、DBIL、TBA、ALT andγ-GT in serum were assayed by fully-auto chemistry analyzer. Results 76 cases of IHS, The positive rate of CMV-IgM is 59.21%, which is significantly higher than the healthy control(3.33%). The positive rate of TOXIgM is 9.21%, but there is no positive cases in the healthy control. In the HIS group, the level of serum TBIL、DBIL、TBA、ALT、γ-GT、IL-6 and TNF-a are higher than the control. Conclusion CMV infection is the most important pathogen of IHS. TOX infection is an important pathogens factor of IHS. The serum level of TBIL、DBIL、TBA、ALT、γ-GT、IL-6 and TNF-a can provide the evidence of the condition and prognosis of IHS.
