一次性根管预备热牙胶即刻充填治疗慢性根尖周炎的临床效果研究

Clinical Study on the Treatment of Chronic Apical Periodontitis by Immediate Filling with One-time Root Canal Preparing Hot

目的:慢性根尖周炎(CAP)是临床上常见的疾病,主要是由于根管内长期存在感染性炎症从而引起慢性炎症性反应,临床上主要表现为肉芽组织形成与牙槽骨破坏。常规根管治疗术虽然能改善患者症状,但是疗效欠佳,预后较差。近年来,一次性根管预备热牙胶即刻充填在部分慢性根尖周炎患者中得到应用,且效果理想。本文主要探讨一次性根管预备热牙胶即刻充填在慢性根尖周炎患者中的治疗效果。方法:取慢性根尖周炎患者90例,随机分为对照组(n=45)和观察组(n=45)。对照组采用常规根管治疗术治疗,观察组采用一次性根管预备热牙胶即刻充填治疗,比较2组临床治疗效果及疼痛情况。结果:观察组前牙、后牙根管预备时间,显著短于对照组;2组根管填前VAS评分比较无统计学差异;观察组填充后6、12个月VAS评分,低于对照组;观察组优良率为93.33%,对照组优良率为66.67%,2组优良率比较差异有统计学意义。结论:与常规根管治疗术治疗相比,慢性根尖周炎患者采用一次性根管预备热牙胶即刻充填治疗效果理想,能减轻患者疼痛,并且该方法安全、可靠,值得推广应用。

Objective Chronic apical periodontitis(CAP) is a common disease in the clinic. It is mainly caused by chronic inflammatory reaction in the root canal. The clinical manifestation is granulation tissue formation And alveolar bone destruction. Conventional root canal therapy can improve the symptoms of patients, but poor efficacy, poor prognosis. In recent years, one-time root canal preparation of hot gutta-percha instantly filled in some patients with chronic apical periodontitis have been applied, and the effect is satisfactory. This study was to investigate the efficacy of disposable root canal preparation of hot guttapercha for immediate filling in patients with chronic apical periodontitis. Methods Ninety patients with chronic periapical periodontitis were randomly divided into control group(n=45) and observation group(n=45). The control group was treated with conventional root canal therapy. The treatment group was treated with one-time root canal preparation. The clinical therapeutic effect and pain were compared between the two groups. Results The time of root canal preparation in the anterior teeth and posterior teeth of the observation group was significantly shorter than that in the control group. There was no significant difference in VAS scores between the two groups. The excellent and good rate was 93.33% in the observation group and 66.67% in the control group. There was significant difference in the excellent and good rate between the two groups. Conclusion Compared with the conventional root canal therapy, the patients with chronic periapical periodontitis can be treated with disposable root canal preparation. The method is safe and reliable. It is worthy of popularization and application.
