无创正压机械通气治疗慢性阻塞性肺疾病急性加重期呼吸衰竭患者疗效观察

Clinic effect of non-invasive positive pressure ventilation in treatment for patients with a cute exacerbation of chronic obstructive pulmonary disease and respiratory failure

目的 :探讨无创正压通气(NIPPV)治疗慢性阻塞性肺疾病急性加重期(AECOPD)并呼吸衰竭的临床疗效。方法 :选择急诊科抢救室162例AECOPD患者随机分为治疗组84例,对照组78例,治疗组在对照组常规药物治疗基础上加用无创正压通气治疗并观察两组治疗前后血气分析、呼吸频率及气管插管率。结果 :治疗组患者血气分析及呼吸频率的改善显著优于对照组,而气管插管率明显低于对照组,差异有统计学意义。结论 :无创正压通气对AECOPD并呼吸衰竭患者疗效显著。

Objective To evaluate the value of non-insive positive pressure ventilation(NIPPV) in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD) with respiratory failure. Methods 162 patients with AECOPD were randomly divied into trial group(84 cases) and control group(78 cases), which respectively, received, conventional medical therapy plus NIPPV and conventional medical therapy only. The differences of blood gas analysis, breath rate and tracheal cannula rate of patients before and after trcatment were compared between the two groups. Results The blood gas analysis, breath rate improved more significantly in the trial group than those in the control groups. meanwhile, the tracheal cannula rate in the trial groups is much lower, and the differences were significant. Conlusion Non-invasive positive pressure ventilation has positive effects on patients of AECOPD with respiratory failure significantly.
