关节松动术联合连续被动运动对膝关节前交叉韧带损伤重建术后早期康复疗效观察 被引量:15

Effect of joint mobilization combined with continuous passive motion on early rehabilitation of anterior cruciate ligament injury in knee joint

摘要 目的 :探讨关节松动术联合连续被动运动(CPM)对膝关节前交叉韧带(ACL)损伤重建术后早期康复的疗效。方法 :选取2014年6月~2016年10月我院收治的65例行膝关节ACL重建术的患者为研究对象。将其中拒绝行关节松动术和CPM康复治疗,而仅接受传统康复锻炼的20例患者归为对照组,其余行关节松动术联合CPM康复治疗的45例患者归为观察组。比较2组患者膝关节活动度,膝关节Lysholm、IKDC和Tegner功能评分,以及视觉模拟疼痛评分(VAS)。结果 :2组患者治疗前膝关节屈-伸、内旋、外旋角度,膝关节Lysholm、IKDC和Tegner功能评分,以及VAS评分比较均无统计学差异。治疗后,观察组患者膝关节屈-伸、内旋、外旋角度均显著大于对照组,膝关节Lysholm、IKDC和Tegner功能评分均显著大于对照组,VAS评分明显低于对照组。结论 :关节松动术联合CPM可有效改善膝关节ACL重建患者膝关节活动度,提高膝关节功能,并减少疼痛,从而促进患者的早日康复。 Objective To investigate the effect of joint mobilization combined with continuous passive motion on early rehabilitation of anterior cruciate ligament injury in knee joint. Methods From June 2014 to October 2016, 65 patients with anterior cruciate ligament injury in knee joint were selected as research objects.20 patients who refused joint mobilization combined CPM rehabilitation therapy, and received only traditional rehabilitation exercise, were classified as the control group.45 patients who received joint mobilization combined CPM rehabilitation therapy were classified as observation group. The knee joint motion, knee function scores(Lysholm, IKDC and Tegner), and visual analogue pain score(VAS) were compared between the 2 groups. Results Before treatment, the angles of knee flexion, extension, internal rotation, and external rotation of the observation group were significantly higher than those of the control group. After treatment, the knee flexion, extension, internal rotation, external rotation angle, knee joint Lysholm, IKDC and Tegner function scores, and VAS scores were not statistically different between the 2 groups. The Lysholm, IKDC and Tegner scores of the observation group were significantly higher than those of the control group. The VAS score of the observation group was significantly lower than that of the control group. Conclusion Joint mobilization combined with CPM can effectively improve the knee joint motion and function after knee joint cruciate ligament reconstruction, and reduce pain, promote the patient’s early recovery.
