小切口白内障手术对白内障伴糖尿病患者角膜内皮细胞及角膜厚度的影响研究

Research the influence of small incision cataract surgery on corneal endothelial cells and corneal thickness in treatment cataract with diabetic patients

目的 :分析小切口白内障手术对白内障伴糖尿病患者角膜内皮细胞及角膜厚度的影响。方法 :选择我院2015年4月～2017年4月收治的108例白内障伴糖尿病患者,按不同治疗方式分为小切口白内障摘除术组及超声乳化白内障摘除术组,每组54例。两组手术前后角膜内皮细胞、角膜厚度、角膜散光度及视力,和并发症发生情况。结果 :术前,两组角膜内皮变异系数、角膜内皮细胞密度、六边形细胞比例、角膜厚度、角膜散光度及视力比较无差异。术后,两组角膜内皮变异系数、角膜厚度、角膜散光度、视力均较术前上升,组间比较无差异;两组角膜内皮细胞密度、六边形细胞比例均较术前下降,组间比较无差异。两组均有干眼症、黄斑囊样水肿、高眼压发生,并发症发生率比较无统计学差异。结论 :小切口白内障手术能够减少白内障伴糖尿病患者角膜内皮细胞,增加角膜厚度,且其影响与超声乳化术相似,且并发症均较少,值得推广运用。

Objective To research the influence of small incision cataract surgery on corneal endothelial cells and corneal thickness in treatment cataract with diabetic patients.Methods 108 cases of cataract patients with diabetes was from April 2015 to April 2017,according to the different treatment was divided into small incision cataract extraction and ultrasonic emulsification cataract extraction group,54 cases in each group.Corneal endothelial cells,corneal thickness,corneal dispersion and vision,and complications occurred before and after the operation of the two groups was compared.Results Before surgery,corneal endothelial variation coefficient,corneal endothelial cell density,hexagonal cell ratio,corneal thickness,corneal dispersion and vision was no different between the two groups.After surgery,the variation coefficient of corneal endothelium,corneal thickness,corneal dispersion,and vision was all higher than before surgery,there was no statistical difference.corneal endothelial cell density and the ratio of hexagonal cells in two groups was decreased than before operation,there was no significant difference between the two groups.Both groups had dry eyes,macular edema and high intraocular pressure,and the incidence of complications was no statistically significant.Conclusion Small incision cataract surgery can reduce cataract and diabetic corneal endothelial cells,increase corneal thickness,but the effect is similar to that of phacoemulsification,and less complications,it is worth promoting.
