三维超声体积自动测量技术联合MRI动态增强对中央型前置胎盘诊断的应用 被引量:6

摘要 目的 :探究三维超声体积自动测量技术联合MRI动态增强对中央型前置胎盘诊断的应用。方法 :选取我院2016年1月~2017年12月收治的总例数68例前置胎盘患者,按照检查方法的不同分为两组,对照组应用传统二维超声技术进行检查,研究组患者则应用三维超声体积自动测量技术联合MRI动态增强技术进行检查。对比两组患者对中央型前置胎盘以及并发胎盘植入进行诊断的敏感性、特异性、误诊率和漏诊率;对比两组患者中央型前置胎盘以及前壁型前置胎盘检出的阳性率;对比两组患者胎盘植入程度的检出率,包括部分性植入、边缘性植入和完全性植入;对比两组患者通过不同方法检出的11~13周胎盘体积大小。结果 :研究组患者对中央型前置胎盘以及并发胎盘植入诊断的敏感性、特异性高于对照组,误诊率和漏诊率低于对照组;研究组患者前壁型前置胎盘检出的阳性率与对照组无显著差异,研究组患者中央型前置胎盘检出的阳性率高于对照组;研究组患者胎盘植入程度的检出率,包括部分性植入、边缘性植入和完全性植入比对照组高;与对照组比较,研究组患者检出的11-13周胎盘体积大小显著高于对照组。结论 :临床研究表明,三维超声体积自动测量技术联合MRI动态增强,能够尽可能早的发现产妇病情,并采取措施,防止产妇病情进一步发展,减少产妇及胎儿因胎盘前置并发胎盘植入造成的死亡情况。 Objective To explore the application of three-dimensional ultrasound volume automatic measurement technology combined with MRI dynamic enhancement in the diagnosis of central placenta previa.Methods 68 cases of placenta previa treated in our hospital in December-2017 January 2016 were divided into two groups according to the different methods of examination.The control group was examined by the traditional two-dimensional ultrasound technique.The patients in the study group were examined by the three-dimensional ultrasonic volume automatic measurement technique combined with MRI dynamic enhancement technique.The sensitivity,specificity,misdiagnosis rate and missed diagnosis rate of two groups of patients with central placenta previa and placenta implantation were compared and the positive rates of central placenta previa and placenta previa in the two groups were compared,and the detection rates of placenta implants in the two groups were compared,including partial implantation and margin.Sexual implantation and complete implantation were compared.The placental volume of 11-13 groups of patients detected by different methods was compared between the two groups.Results The sensitivity and specificity of the patients in the study group for the diagnosis of central placenta previa and placenta implantation was higher than that of the control group.The misdiagnosis rate and missed diagnosis rate were lower than the control group.The positive rate of the positive placenta previa in the study group was not significantly different from the control group,and the center of the study group was the central part of the study group.The positive rate of placenta previa was higher than that of the control group.The detection rate of placenta implants in the study group was higher than that of the control group,including partial implantation,marginal implantation and complete implantation;the 11-13 week placental volume of the study group was closer to the normal value than the control group.Conclusion Clinical studies have shown that placenta previa itself is not worth paying attention to,but placenta previa has a large probability of causing serious consequences,namely placental implantation.Once the placenta is implanted,the probability of a pregnant woman with a placenta implantation,the probability of a bad pregnancy results greatly,the risk of maternal and fetal occurrence is also significantly increased,three-dimensional ultrasound volume from the volume.The dynamic enhancement of MRI combined with dynamic measurement is of great significance to the diagnosis of placenta previa.In clinic,it is of great significance to prevent placenta implantation for the possible appearance of placenta previa.It is possible to find the maternal condition as early as possible,and take measures to prevent the further development of the maternal condition and reduce the maternal and fetal fetus.Death due to placenta previa complicated with placenta accreta.
