VEGF和MMP-9蛋白在口腔扁平苔藓癌变发展中的作用 被引量:2

摘要 目的 :探究VEGF和MMP-9蛋白在口腔扁平苔藓癌变发展中的作用。方法 :选取本院于2016年6月~2017年8月收治的40例口腔扁平苔藓(OLP)患者作为研究组,再选同期外伤手术患者30例作为对照组。研究组(n=40)将患者的病变黏膜组织进行病理检查以及PAS染色,对照组(n=30)将患者的正常黏膜组织进行对应研究。对比分析VEGF和MMP-9蛋白在口腔扁平苔藓癌变发展中的作用。结果 :根据研究结果显示,40例OLP患者均存在角化过度,其中有5例轻微局灶性角化不全(12.50%)、有35例颗粒层楔形增厚(87.50%)。全部患者均存在基底细胞崩解或者消失,淋巴细胞浸润至真皮浅层。其中有18例胶样小体(45.00%),多见于真皮浅层、基底细胞层和棘细胞层当中,呈现出扁平苔藓或者红斑狼疮。40例OLP患者中有82.50%患者由淋巴细胞侵入真皮浅层并扩展到了基底细胞内;17.50%患者由炎细胞侵入真皮浅层或者棘细胞层,其水肿现象十分明显,并且细胞间桥存在拉长或断裂现象。起码有70.00%患者的血管数目变多,血管扩张等现象,30%的患者虽然没有发现血管结构变化,但可以明显发现红细胞增多。研究组的VEGF、MMP-9水平明显高于对照组;研究组的VEGF/GAPDH、MMP-9/GAPDH明显高于对照组;研究组的MMP-9/GAPDH明显高于对照组。结论 :VEGF和MMP-9蛋白在口腔扁平苔藓癌变发展着重要作用。当VEGF水平较高时,OLP患者真皮浅层的血管扩张或增多,同时可能会有局部缺血缺氧的现象发生;当MMP-9水平较高时,可将基底膜带降解,从而减少存活素的产生,并促进T淋巴细胞入侵真皮浅层,故MMP-9水平可能与TNF-α存在密切的关系。 Objective To investigate the role of VEGF and MMP-9 in the carcinogenesis of oral lichen planus.Methods Forty patients with oral lichen planus(OLP)cancer treated in our hospital from June 2016 to August 2017 were enrolled as study group.Thirty patients with traumatic surgery during the same period were selected as control group.The study group(n=40)patients with mucosal lesions was given pathological examination and PAS staining on the skin lesions of the patients,and the normal mucosa lesions of the control group(n=30)was given corresponding study.The role of VEGF and MMP-9 in the carcinogenesis of oral lichen planus was compared and analyzed.Results According to the results of the study,all 40 patients with oral lichen planus(OLP)had hyperkeratosis,including 5 cases of focal focal keratosis(12.50%)and 35 cases of granular layer wedge thickening(87.50%).All patients had basal cell disintegration or disappearance,lymphocyte infiltration into the superficial dermis.Among them,18 cases(45.00%)were found in the dermis,basal cell layer and spine cell layer,showing lichen planus or lupus erythematosus.82.50%of 40 patients with oral lichen planus(OLP)invaded the superficial layer of the dermis by lymphocytes and spread into the basal cells;17.50%of the patients invaded the superficial layer of the dermis or the spinous cell layer by inflammatory cells,and the edema phenomenon was very obvious,and the intercellular bridges elongated or broken.At least 70.00%of the patients had more blood vessels,vasodilation and other phenomena.Although 30%of the patients did not find any changes in the vascular structure,they could obviously find that the number of red blood cells increased.The levels of VEGF and MMP-9 in the study group were significantly higher than those in the control group.The levels of VEGF/GAPDH and MMP-9/GAPDH in the study group were significantly higher than those in the control group.The MMP-9/GAPDH in study group was significantly higher than that in control group.Conclusion The VEGF and MMP-9 proteins play an important role in carcinogenesis of oral lichen planus(OLP).When the level of VEGF is higher,the superficial dermal vessels in patients with oral lichen planus expand or increase in blood vessels,meanwhile there may be ischemia and hypoxia.When the level of MMP-9 is higher,the basilar membrane may be degraded,Thus reducing the production of survivin and promoting the invasion of T lymphocytes into the dermal superficial layer.Therefore,the level of MMP-9 may be closely related to TNF-α.
