超声心动图评价老年退行性心脏瓣膜病的临床特征

Echocardiographic evaluation of clinical features of senile degenerative valvular heart disease

目的 :研究老年退行性心脏瓣膜病经胸超声心动图显像特征。方法 :回顾性分析524例年龄60～89岁老年患者的临床资料,观察老年退行性心瓣膜病患者检出率及不同年龄段检出率,并对主动脉瓣、二尖瓣、三尖瓣及肺动脉瓣等各瓣膜情况,进行综合性分析以及对心室重构的影响。结果 :本组病例老年退行性心脏瓣膜病发病率为33.2%,患病数为174例。其发病率随着年龄增加而增加;退行性病变累及瓣膜及其功能改变情况:单纯主动脉瓣发病率显著高于单纯二尖瓣及主动脉瓣联合二尖瓣;与非SDHVD对照组比较SDHVD组其心室重构发生变化,其中以左心结构改变明显。结论 :老年退行性心脏瓣膜病以主动脉瓣病变多见,瓣膜功能失常多以瓣膜关闭不全为主。SDHVD患者有左心室重构倾向并伴有心功能减退。

Objective To study the characteristics of transthoracic echocardiography in elderly patients with degenerative valvular heart disease.Methods A retrospective analysis of 524 cases of patients aged 60-89 years with echocardiography data,the detection rate of elderly patients with degenerative valvular heart disease and the detection rate of different age groups,and aortic valve,mitral valve,three The valvular and pulmonary valves and other conditions of the valve,for comprehensive analysis and impact on ventricular remodeling.Results The incidence of degenerative valvular heart disease in this group was 33.2%,174 cases.The incidence increased with age;Degenerative lesions involving valve and its functional changes:the incidence of simple aortic valve was significantly higher than that of mitral and aortic valve combined with mitral valve;After comparison with the non-SDHVD control group,the results of this study showed that the ventricular remodeling changed in the SDHVD group,and the left heart structure changed significantly.Conclusion The aged degenerative heart valve disease grows with age.The echocardiographic findings are mainly caused by aortic valve involvement,followed by mitral valve.Valve dysfunction is mainly caused by valvular insufficiency.Left ventricular remodeling tendency in SDHVD patients,with left ventricular structural changes and cardiac dysfunction.
