维生素D对COPD稳定期患者血清TNF-α、IL-6及肺功能的影响

Effects of Vitamin D on serum levels of TNF-α、IL-6 and Pulmonary Function in patients with Chronic Obstructive Pulmonary Disease at stable stage

目的 :探讨维生素D对COPD稳定期患者血清TNF-α、IL-6及肺功能的影响。方法 :选取COPD稳定期患者80例分为常规治疗组(A组)和维生素D干预组(B组)各40例,选取同期门诊健康体检者40例为对照组(C组);A组:信必可都保164.5μg/次吸入2/日+茶碱控释片0.1g/次饭后口服2/日;B组:在A组基础上加用维生素D3 800IU/次口服1/日;C组不处理。观察并比较各组25(OH)D、TNF-α、IL-6及肺功能的变化。结果 :①治疗前A、B两组TNF-α、IL-6高于C组,25(OH)D、FEV1、FEV1%低于C组,而A、B组间各指标无明显差异;②治疗后B组25(OH)D较前明显上升;③治疗后A、B组各指标较前好转,但B组改善更明显。结论 :COPD稳定期患者气道仍存在慢性炎症,维生素D能降低炎症因子水平减轻炎症反应,并能在一定程度上改善肺功能。

Objective To investigate the effects of vitamin D on serum TNF-α,IL-6 and lung function in patients with stable COPD.Methods Eighty patients with stable COPD were divided into the conventional treatment group(group A)and the vitamin D intervention group(group B),40 cases were selected,and 40 healthy subjects in the same period were selected as healthy control group(group C);group A:Budesonide formoterol powder inhaler 164.5μg/inhalation 2/day+theophylline controlled release tablets 0.1 g/oral 2/day after meals;Group B:add vitamin D3 800 IU/oral on the basis of group A 1/day;Group C does not process.The changes of serum 25(OH)D,TNF-α,IL-6 and lung function were observed and compared.Results①The levels of TNF-αand IL-6 in group A and group B were higher than those in group C before treatment,and 25(OH)D,FEV1 and FEV1%were lower than group C.②There were no significant differences in 25(OH)D,TNF-α,IL-6,FEV1,and FEV1%between groups A and B before treatment.There was no significant difference between the indicators in groups A and B.③After treatment,the levels of TNF-α,IL-6,FEV1 and FEV1%in group A and group B were better than before,but the improvement in group B was more significant.Conclusion Chronic inflammation is still present in the airway of patients with stable COPD.Vitamin D can reduce the level of related inflammatory factors and reduce inflammation,and can improve lung function to some extent.
