维持性血液透析患者伴发代谢综合征的危险因素分析

Influencing factors of metabolic syndrome among patients with maintenance hemodialysis

目的 :了解维持性血液透析(MHD)患者代谢综合征(MS)的发病情况分析相关影响因素。方法 :泸州市三家医院的305例MHD患者,根据国际糖尿病联盟MS诊断标准分为MS组和非MS组,采用单因素分析法比较两组临床资料,多因素logistic回归分析法分析MS发病的影响因素。结果 :泸州市MS的发病率为39.6%,两组间性别、合并心血管疾病(CVD)、腰围、体重、透析龄、单室尿素清除指数(spKt/v)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、空腹血糖(FPG)、超敏C反应蛋白(hs-CRP)、血清铁、白细胞、吸烟、运动相比较,差异均具有统计学意义。多因素logistic回归分析结果提示hs-CRP、血清铁、白细胞、吸烟与MS发生呈正相关,运动、透析龄与MS发生呈负相关。结论 :泸州市MHD患者MS发病率较高,合并CVD风险较大,hs-CRP、血清铁、白细胞、吸烟是MS发生的危险因素,运动、透析龄是MS发生的保护因素。

Objective To investigate the prevalence of metabolic syndrome(MS)in maintenance Hemodialysis(MHD)patients,and to analyze the related influencing factors.Methods Totally 305 cases of MHD patients were divided into the MS group and non-MS group according to International Diabetes Federation,Single factor analysis was used to compare the clinical data of two groups,and the Multi-factor Logistic regression analysis was used to analyze the influencing factors of MS.Results The prevalence of metabolic syndrome in maintenance Hemodialysis patients of Luzhou city was 39.6%,There were significant differences in sex,combined cardiovascular disease(CVD),waist circumference,weight,dialysis age,the single chamber urea clearance index(spKt/V),triglyceride(TG),high density lipoprotein(HDL-C),fasting plasma glucose(FPG),hypersensitivity CRP(hs-CRP),serum iron,white blood cells,smoking,exercise between these two groups,resectively).Multivariate logistic regression analysis showed that hsCRP,serum iron,white blood cells and smoking were positively correlated with MS,while exercise and dialysis age were negatively correlated with MS.Conclusion The incidence of MS is higher in MHD patients in Luzhou,and the risk of CVD is higher,hs-CRP,serum iron,white blood cells and smoking are risk factors for MS.Exercise and dialysis age are protective factors for MS.
