雾化吸入布地奈德联合免疫球蛋白治疗新生儿感染性肺炎的效果及对血清MCP-1、cTnT水平的影响研究 被引量:18

摘要 目的 :探究雾化吸入布地奈德联合免疫球蛋白治疗新生儿感染性肺炎的效果及对血清MCP-1、c TnT水平的影响。方法 :2017年1月~2018年1月间新生儿感染性肺炎患儿共92例,随机分为两组,各46例。对照组使用布地奈德治疗,观察组在对照组基础上联合免疫球蛋白治疗。比较两组患者治疗后的临床疗效、临床指标消退时间和不良反应发生情况。检测并比较两组患者治疗前后血清免疫球蛋白和MCP-1、cTnT水平。结果 :两组的临床疗效有显著差异,观察组的治疗有效率显著高于对照组。观察组的呼吸困难消退、咳嗽消失、肺啰音消失以及住院部时间均显著低于对照组。治疗前两组血清IgG和IgM水平比较无显著差异,治疗后均显著升高,治疗后观察组血清IgG和IgM水平均显著高于对照组。治疗前两组血清MCP-1、cTnT水平比较无显著差异,治疗后均显著下降,治疗后观察组血清MCP-1、cTnT水平均显著低于对照组。两组不良反应发生情况无显著差异。结论 :布地奈德联合免疫球蛋白对于新生儿感染性肺炎的患儿具有更好的疗效,并且联合应用可以调节患儿免疫功能,并降低对心肌细胞的损伤,可以提高预后。 Objective To investigate the effect of nebulized inhaled budesonide combined with immunoglobulin on neonatal infectious pneumonia and its effect on serum MCP-1 and cTnT levels.Methods From January 017 to January 2018,a total of92 children with neonatal infectious pneumonia were randomly divided into two groups,46 cases each.The control group was treated with budesonide,and the observation group was combined with immunoglobulin treatment on the basis of the control group.The clinical curative effect,time for regression of clinical indicators and occurrence of adverse reactions were compared between the two groups after treatment.The levels of serum immunoglobulin,MCP-1,and cTnT before and after treatment were measured and compared between the two groups.Results The clinical efficacy of the two groups was significantly different.The treatment efficiency of the observation group was significantly higher than that of the control group.Obstructed breathing difficulty,disappearance of cough,disappearance of pulmonary rales and hospitalization time were significantly lower in the observation group than in the control group.There was no significant difference in serum IgG and IgM levels between the two groups before treatment,and they both increased significantly after treatment.After treatment,serum IgG and IgM levels in the observation group were significantly higher than those in the control group.There was no significant difference in the serum levels of MCP-1 and cTnT between the two groups before treatment.After treatment,the levels of MCP-1 and cTnT were significantly lower.After treatment,serum levels of MCP-1 and cTnT were significantly lower in the observation group than in the control group.There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Budesonide combined with immunoglobulin for neonatal infectious pneumonia in children with better efficacy,and combined application can regulate the immune function of children,and reduce damage to myocardial cells,can improve the prognosis.
