早期联合检测PCT与CRP水平早期鉴别支原体感染肺炎和细菌性感染肺炎患儿的价值探讨

Value of Early Combined Detection of PCT and CRP Levels in Early Diagnosis of Mycoplasma Pneumoniae and Bacterial Pneumonia in Children

目的 :分析早期联合检测PCT与CRP水平早期鉴别支原体感染肺炎和细菌性感染肺炎患儿的价值。方法 :回顾性分析我我院2012年1月~2017年1月收治50例支原体肺炎感染患儿,根据支原体肺炎类型不同分为支原体感染组与细菌性感染组。其中26例为支原体感染肺炎患儿,纳入支原体感染组;另有24例为细菌性感染肺炎患儿,纳入细菌性感染组。纳入同期25例健康儿童,作为健康对照组。三组受试者入院治疗前抽取外周静脉血5mL,使用全自动生化学发光分析仪及相关试剂盒检测受试者PCT。使用全自动生化分析及相关试剂盒检测CRP。收集患儿与健康受试者恢复期血液样本,重复上述检测内容。应用全自动生化分析测定心肌酶谱CK、AST、LDH、α-HBDH。结果 :三组PCT、CRP水平比较差异有统计学意义。细菌性感染组CT、CRP水平显著高于支原体感染组、健康对照组,支原体感染组CT、CRP水平显著高于健康对照组。急性期、恢复期时细菌性感染组AST、CK、LDH、α-HBD水平显著高于支原体感染组。联合检测敏感性及特异性高于单纯血清PCT、CRP检测。结论 :联合检测PCT、CRP水平有利于提高支原体感染肺炎和细菌性感染肺炎鉴别的正确性与灵敏性,值得推荐使用。

Objective To investigate the value of early combined detection of PCT and CRP levels in early diagnosis of mycoplasma pneumoniae and bacterial pneumonia in children.Methods 50 cases of mycoplasma pneumonia in our hospital from January 2012 to January 2017 were analyzed retrospectively,then divided into mycoplasma infection group(n=26)and bacterial infection group(n=24).Another 25 health children were selected as control group.5 ml peripheral venous blood before admission of three groups were collected to detect the PCT and CRP by the automatic biochemical analysis and related kits.Then the blood samples during recovery period of the three groups were collected again to detected the enzyme CK,AST,LDH,andα-HBDH by automatic biochemical analysis.Results The differences of PCT and CRP levels in the three groups were statistically significant.The levels of CT and CRP in a descending order were bacterial infection group,mycoplasma infection group and control group,with statistic difference.The levels of AST,CK,LDH,andα-HBD in the bacterial infection group were significantly higher than those in the mycoplasma infection group at the acute and recovery phases.The sensitivity and specificity of combined detection were higher than the single detection.Conclusion Combined detection of PCT and CRP levels is beneficial to improve the diagnosis accuracy and sensitivity of mycoplasma pneumoniae and bacterial pneumonia,which is worthy of recommendation.
